Abstract

RÉSUMÉ
Buts. Décrire les différentes indications de la fibroscopie œsogastroduodénale (FOGD) et le profil des lésions retrouvées à l’endoscopie haute. Patients et méthodes. Étude rétrospective réalisée à partir des résultats de compte rendus d’endoscopie haute dans le service d’Hépato-Gastro-Entérologie du CHU Campus de Lomé du 1er septembre au 31 Décembre 2018 chez les patients âgés de moins de 18 ans. Résultats. 304 endoscopies hautes ont été réalisées durant la période d’étude dont 50 chez les patients âgés de 18 ans au plus, soit une fréquence de 16,4%. L’âge moyen des patients était de 15,63 ans avec les extrêmes (7-18 ans). Aucun nourrisson n’avait réalisé l’endoscopie haute pendant la période d’étude. On notait une prédominance féminine (ratio = 0,82). L’indication de l’endoscopie la plus fréquente était l’épigastralgie (50%), suivie du syndrome ulcéreux (22%) et de l’hématémèse (12%). On notait deux cas de tentative d’autolyse et aucune ingestion caustique. Les différentes pathologies retrouvées étaient essentiellement inflammatoires gastrique (37,8%), duodénale (2,8%) et œsophagienne (1,4%). Le reflux biliaire duodénogastrique associé ou non à une gastropathie était la lésion la plus fréquente. Les lésions endoscopiques hautes d’hypertension portale représentaient 15,4%. Il y a eu  un seul cas d’ulcère bulbaire et aucune lésion tumorale. L’examen endoscopique était normal chez 10 patients. Conclusion. Le reflux biliaire duodénogastrique et les pathologies inflammatoires gastriques sont les lésions les plus fréquemment retrouvées au cours des endoscopies pédiatriques. Une sensibilisation des pédiatres s’avère indispensable car la demande de l’examen endoscopique n’est pas encore une pratique courante au Togo.

ABSTRACT
Aim. To describe the indications of upper GI endoscopy and the pattern of lesions found on upper endoscopy. Patients and methods. This was a cross sectional retrospective study carried out from the results of upper GI endoscopy reports in the Hepato-Gastroenterology Department of the CHU Lomé Campus from September 1st to December 31st 2018 in patients aged 18 years or less. Results. 304 upper endoscopies were performed during the study period. Among them, there were 50 patients aged 18 years or less (16.4%). The mean age of the patients was 15.63 years with the extremes (7-18 years). Most patient were females (sex-ratio F/M= 0.82). The most frequent indication for endoscopy was epigastralgia (50%), followed by ulcerative syndrome (22%) and hematemesis (12%). We found two cases of attempted self slaughter and no caustic ingestion. Diagnosed pathologies found mainly inflammatory lesions, gastric (37.8%), duodenal (2.8%) or esophageal (1.4%). Duodenogastric biliary reflux associated or not with gastropathy was the most common lesion. Upper GI endoscopic lesions of portal hypertension were found in 15.4 % of cases. There was only one case of bulbar ulcer and no tumor lesion. Endoscopic examination was normal in 10 patients.  Conclusion. Duodenogastric biliary reflux and inflammatory gastric diseases are the most common lesions during upper GI endoscopy of young patients. Awareness raising actions directed to pediatricians in Togo are mandatory to optimize the request for endoscopic examination.
