Abstract

PURPOSE / AIM
To evaluate the introduction of lumbo-sacral injections of corticosteroids in Yaoundé five years after a training session on interventional radiology
METHODS
We reviewed data on 524 lumbo-sacral injections of corticosteroids performed between April 2006 and March 2011 in 267 patients (183F/84M) aging 34 to 78 years particularly clinical features, the technique used, the duration of the injection, the technical success, the side-effects and the costs incurred by the patients.
RESULTS
The two main indications were mechanical lumbar pain associated with multiple degenerative disk and degenerative listhesis (129 women) and sciatic neuralgia due to posterior, lateral or foraminal disk hernia, not responding to well conducted medical treatment (96 patients). Procedures were performed under scopy (93 patients) and CT scan guidance (174 patients). Patients received 1 to 4 injections frequently combining a double injection facet, epidural and/or intra dural. The mean cost of commodities and user fees ranged XAF 49,250 to 58,250. Patients suffering from disk hernia were the most dramatically relieved (67/83 patients); 117 patients agreeing to use a lumbar belt following the injection experienced the lengthier pain relief.
CONCLUSION
This retrospective analysis highly suggests the need for a clinical trial to clarify the most appropriate indications.

OBJECTIFS
Évaluer l’introduction des infiltrations rachidiennes cortisonées à Yaoundé cinq ans après la tenue d’une formation sur la radiologie interventionnelle antalgique
MÉTHODES
Nous avons analysé les données relatives aux 524 infiltrations rachidiennes lombosacrées cortisonées réalisées entre avril 2006 et Mars 2011 chez 267 patients (183F/84M) âgés de 34 à 78 ans notamment le profil clinique, la technique utilisée, la durée du geste, le succès technique, la survenue de complications, les coûts encourus par les patients.
RÉSULTATS
Les deux principales indications étaient la lombalgie mécanique associée à une discopathie dégénérative étagée avec un listhésis d’origine dégénérative zygo apophysaire (129 patientes) et la lombo-radiculalgie en relation avec une hernie discale postéro-latérale ou foraminale en échec thérapeutique (96 patients). Les procédures ont été réalisées sous scopie (93 patients) et sous scanographie (174 patients). Les patients ont reçu entre 1 et 4 injections associant souvent une double injection zygo apophysaire, épidurale et/ou intra durale. En moyenne, le matériel et le geste coûtait 49 250 à 58 250 FCFA. Les patients avec une hernie discale ont décrit l’amélioration clinique la plus nette (67/83 patients) ; 117 patients ayant accepté le port d’une ceinture de soutien lombaire ont expérimenté des accalmies plus longues
CONCLUSION
Cette analyse rétrospective suggère fortement la nécessité d’un essai clinique qui permettra de clarifier les indications les plus appropriées.
