ABSTRACT
Thoracic wounds by the traditional devices are lesions affecting the chest wall and/or thoracic viscerae. Usually the devices used are: dagger, cutlass, knife, screwdriver. But some other agent like sharp spear can be also used mainly during popular justice molestation or social violence.
Authors reported a case of perforating thoracic wound secondary to a spear. The objective of this work was to describe the difficulties of management of this type of accident without a formal system of emergency. The clinical presentation was hemopneumothorax associated with sepsis. Thoracotomy, drainage and infection treatment are still the principle of management.

RÉSUMÉ
Les plaies thoraciques par arme blanche sont des lésions intéressant la paroi et/ou le contenu viscéral du thorax. Les agents vulnérants les plus fréquent sont : le poignard, le coupe-coupe, le couteau et le tournevis. La lance apparaît dans des circonstances de violence rurale.
Les auteurs rapportent un cas de plaie perforante thoracique causée par un coup de lance, l’objectif étant de décrire les difficultés de la prise en charge de ce type d’accident en l’absence d’un système formel de gestion des urgences. Le tableau clinique était dominé par un hémo-pneumothorax associé à un sepsis. La thoracotomie, le drainage des épanchements et le traitement de l’infection demeurent le principe de la prise en charge.
