Ainsi, en Afrique sub-saharienne, la plupart des femmes sont infectées par leur conjoint, dans le cadre des relations sexuelles conjugales. Objectif. Évaluer le risque conjugal d’exposition au VIH chez les patients suivis dans le service de maladies infectieuses du CHU du Point-G, Mali. Méthodologie. Il s’agit d’une étude prospective, transversale et descriptive portant sur les personnes vivant avec le VIH/SIDA (PVVIH) suivies dans le service de maladies infectieuses de janvier à juin 2014. Les données portant sur les facteurs de risque de la transmission sexuelle du VIH, sociodémographiques et les informations relatives à leur vie de couple ont été cueillies à partir d’un questionnaire lors des entrevues individuelles. Résultats. Nous avions colligé 110 patients dont 37 hommes (33.6%) et 73 femmes (66.40%), parmi lesquelles 31 (28,20%) étaient dans un régime polygamique. Les rapports sexuels extraconjugaux ont été notés chez 25 patients (22,70%). Les rapports sexuels n’étaient pas protégés chez 73 patients et parmi eux, 26 patients avaient une charge virale détectable. Parmi ces 26 patients, neuf patients avaient eu des rapports sexuels extraconjugaux non protégés. La sérologie VIH était positive chez 68,4% parmi 53 conjoints dépistés. 49 patients (44.55%) avaient un taux de lymphocytes TCD4 compris entre 351 et 500 et 35 patients (31.82%) avaient un taux de lymphocytes TCD4 supérieurs à 500. Conclusion. Deux tiers des PVVIH ont des rapports sexuels non protégés avec leurs conjoints souvent parce que le conjoint n’a pas été informé, parfois parce qu’elle refuse ou est déjà lui séropositif. Près d’un patient sur quatre a des rapports extraconjugaux. Le VIH/SIDA constitue toujours un sujet tabou dans notre société. Il est important de renforcer la communication sur le changement de comportement positif afin d’améliorer l’utilisation des mesures préventives et faciliter la prise en charge des PVVIH.</p><p>ABSTRACT<br />Introduction. The transmission of HIV through heterosexual sex has been one of the main ways to the origin of the global AIDS epidemic. In Sub-Saharan Africa, most women are infected in the context of marital sex by their spouses. Objective. The aim of this study was to evaluate HIV conjugal exposure risks among seropositive patients of the Infectious and Tropical Diseases ward of Point-G Teaching Hospital of Bamako, Mali. Methods. We conducted a descriptive cross-sectional study. Our study group was constituted of HIV infected patients followed in the Infectious and Tropical Diseases ward at Point-G Teaching Hospital of Bamako from January to June 2014. HIV sexual transmission risk factors, demographics and information related to their sexual life data were collected by a questionnaire from individual interviews. Results. 110 HIV-positive patients taking antiretroviral therapy were included. There were 37 men (33.6%) and 73 women (66.40%), and 31 women (28.20%) were in polygamous regime. 25 patients (22.70%) acknowledged extramarital sex. Sexual relations were unprotected in 73 patients, among whom 26 patients had a detectable viral load. Nine patients had unprotected extramarital sex. HIV serology was positive in 68.4% of 53 spouses screened. The value of the CD4 cell count was between 351 and 500 cells/mm3 in 44.55% of the cases and higher than 500 cells/mm3 (31.82%). Conclusion. About two thirds of HIV infected patient have unprotected sex with their spouses and a about a quarter of them have extramarital relationships. More than a quarter of our patients had not informed their partners about their HIV positive status. HIV/AIDS is still a taboo in our society.
