Second Cervical Nerve Schwanoma : A Report of Two Cases with Review of Literature

RÉSUMÉ
Le schwannome du deuxième nerf cervical (C2) est une tumeur mésenchymateuse développée exclusivement à partir des cellules de la gaine de schwann qui entourent les fibres nerveuses de ce nerf. Il se caractérise par une latence clinique et un diagnostic tardif. Nous rapportons deux cas de schwannome du nerf C2. Il s’agissait de deux femmes, une de 24 ans tétraplégique et l’autre de 40 ans ayant un syndrome rachidien, après leur accouchement. L’Imagerie par Résonance Magnétique (IRM) mettait en évidence une masse tissulaire intrarachidienne en forme de C autour de la moelle épinière entre C1 et C3, de contours polycycliques, présentant un hyposignal en T1, un hypersignal homogène en T2 et rehaussée de manière tardive et progressive après injection de gadolinium. Le schwannome du nerf C2 est une entité rare mais possible, pouvant être diagnostiqué chez les femmes en période postpartum. Une exérèse chirurgicale complète extra capsulaire peut permettre une récupération totale des patients. Il est important de prendre en compte cette possibilité dans la démarche diagnostique des tuméfactions cervicales survenant après un accouchement.
ABSTRACT
The schwannoma of the second cervical nerve (C2) is a mesenchymal tumor developed exclusively from the Schwann cells surrounding the nerve fibers of this nerve. It is characterized by clinical latency and late diagnosis. We report two cases of C2 nerve schwannoma. These were two women, one of 24 years old tetraplegic and the other of 40 years old with a spinal syndrome, after their delivery. Magnetic Resonance Imaging (MRI) showed an intraspinal tissue mass in the shape of a C around the spinal cord between C1 and C3, with polycyclic contours, presenting a hypointense signal in T1, a homogeneously hyperintense signal in T2 and enhanced late and progressively after injection of gadolinium. Schwannoma of the C2 nerve is a rare but possible entity, which can be diagnosed in postpartum women. Complete extracapsular surgical resection may allow for full recovery of patients. It is important to consider this possibility in the diagnostic process of cervical swellings occurring after childbirth.
