Management of Scrotoschisis: A Case Report From Douala

RÉSUMÉ
Le scrotoschisis est une malformation congénitale rare où le scrotum n'est pas complètement fermé, laissant les testicules exposés à l'extérieur du corps. Nous rapportons un cas de scrotoschisis droit traité au service de chirurgie de l’hôpital Ad Lucem de Bonamoussadi à Douala au Cameroun en mai 2023. L’enfant était né de manière eutocique d’une grossesse à terme avec un poids de 3200 g. Deux échographies réalisées pendant la grossesse n’avaient rien révélé. À la naissance, nous avons objectivé un défect scrotal droit ovalaire de 2.5 cm/1 cm sur la face latérale droite. Les berges étaient nettes et ne saignant pas au contact, extériorisant un testicule droit recouvert de la fibrine. Une chirurgie sous anesthésie locale a été réalisée suivie d’une antibioprophylaxie. Les suites opératoires ont été simples : sortie au 5ème jour, cicatrisation complète en première intention au 14ème jour et testicules cliniquement normaux. Ce cas montre l'importance d'un diagnostic précoce et d'un traitement adapté pour assurer de bons résultats cliniques.
ABSTRACT
Scrotoschisis is a rare congenital malformation in which the scrotum is not completely closed, leaving the testes exposed outside the body. We report a case of right scrotoschisis treated in the surgical department of the Hospital Ad Lucem de Bonamoussadi in Douala, Cameroon in May 2023. The child was born eutocially from a full-term pregnancy with a weight of 3200 g. Two ultrasound scans carried out during the pregnancy revealed nothing. At birth, we objectified a 2.5 cm/1 cm oval right scrotal defect on the right side. The banks were clean and not bleeding on contact, externalizing a right testicle covered with fibrin. Surgery under local anaesthesia was followed by antibioprophylaxis. The operative sequelae were simple: exit on the 5th day, complete first-line healing on the 14th day and clinically normal testes. This case demonstrates the importance of early diagnosis and appropriate treatment to ensure good clinical outcomes.
