Abstract


RÉSUMÉ
Contexte. La drépanocytose est la première maladie génétique au monde. Les complications cardiorespiratoires sont les principales causes de mortalité chez le drépanocytaire adulte. La situation dans notre pays est mal connue. Notre objectif était de décrire le profil épidémiologique des complications cardiaques dans un groupe de patients suivis à Yaoundé. Méthodologie. L’étude était transversale. Les participants volontaires âgés d’au moins 15 ans ont été recrutés au sein des associations de lutte contre la drépanocytose à Yaoundé. L’entretien des participants et les différentes explorations para cliniques se sont faites dans le service de cardiologie du CHUY et le service de radiologie de l’HCY. L’analyse statistique descriptive s’est faite à partir : i) des antécédents médicaux, ii) des paramètres anthropométriques, iii) des symptômes et signes cliniques, iv) des résultats de l’échocardiographie, de l’ECG de repos, de la radiographie du thorax et du test de marche de 6 minutes. L’analyse statistique a été faite grâce au logiciel Epi Info version 7. Résultats. Nous avons recruté 39 participants pour cette étude dont 3 ont été exclus. L’âge moyen des était de 28,1 ans (±10, 7) avec un sexe ratio de 0,72 en faveur des femmes. Tous les participants présentaient au moins une complication cardiaque. Sur le plan des explorations para cliniques les principaux résultats retrouvés étaient : 97 % d’échocardiographies anormales avec 91,4% d’atteintes valvulaires, 69 % d’ECG de repos anormaux avec 42 % de QTc allongé. A la radiographie du thorax, 13,3% des participants avaient une cardiomégalie. Le test de marche de 6 minutes était anormal chez 2/3 des participants. Conclusion. Le profil épidémiologique des complications cardiaques du drépanocytaire adulte dans notre population d’étude est le même que celui retrouvé dans la littérature, avec une prévalence de 100%. Cependant aucune hypertension artérielle pulmonaire n’a été retrouvée.


ABSTRACT
Context. Sickle cell anemia is the first genetic disease in the world. Cardiorespiratory complications are the main causes for death in the adult patient. The situation in our country is poorly known. Our purpose was to describe the epidemiological profile of cardiac complications in a group of patient followed up in Yaounde. Methodology. The study was transversal. Voluntary participants aged at least 15 were chosen within sickle cell control associations in Yaounde. Interviews of participants and various paramedical consultations were carried out in the cardiology unit of the CHUY and in the radiology unit of the HCY. The descriptive statistical analysis considered: i) medical history, ii) anthropometric parameters, iii) clinical signs and symptoms, iv) results of echocardiography, ECG at rest, thorax radiography and the six minute walk test. The statistical analysis was done using the EPI Info software version 7. Results. We recruited 39 participants for this study, of which 3 were excluded. The average age was 28.1 (±10, 7) with a gender ratio of 0, 72 in favor of women. All participants had at least one cardiac complication. On the aspect of para clinical explorations, the main results were: 97 % of abnormal echocardiographies with 91,4% valve pathology, 69 % abnormal ECG at rest with 42 % QTc when lying down. On the radiography of the thorax, 13, 3% of participants had a cardiomegaly. The 6 minute walk test was abnormal in 2/3 of participants. Conclusion. The epidemiological profile of cardiac complications in the adult sickle cell patient in our study population is the same as in the literature with a prevalence of 100%. However no pulmonary arterial hypertension was found.
