Abstract


RÉSUMÉ
Introduction. La polykystose rénale autosomique dominante (PKRAD) est la néphropathie héréditaire la plus fréquente au monde. Le but de notre étude était d’étudier les caractéristiques épidémiologiques, cliniques, paracliniques, thérapeutiques et évolutives des cas de PKRAD en milieu semi-urbain au Sénégal. Matériels et méthodes. Il s’agissait d’une étude prospective, multicentrique, transversale s’étendant sur 3 ans du 01 janvier 2015 au 31 décembre 2017. Ont été inclus dans l’étude, tous les patients hospitalisés ou reçus en consultations, dans trois services de néphrologie pour une PKRAD et âgés de plus de 15 ans. Résultats. Sur les 03 années d’étude, 25 cas de PKRAD ont été colligés soit une prévalence de 0,30%. L’hypertension artérielle était présente chez tous nos patients. De gros reins palpables étaient retrouvé chez 22 patients (85%) suivis des douleurs lombaires chez 21 patients (60%). La majorité des patients avait une insuffisance rénale au moment du diagnostic. Les kystes extra-rénaux se trouvaient essentiellement au niveau du foie chez 8 patients soit 68 %. Le taux d’hémoglobine moyen était de 9,91 g/dl. Une étude génétique a été réalisée chez 2 patients (8%) et a confirmé une mutation du gène PKD1. Quatre patients (16%) avaient bénéficié de l’hémodialyse chronique. Un seul patient avait bénéficié d’un traitement à base de Vaptan (4%). L’évolution était marquée par la survenue de 4 décès soit 16%. Conclusion. La PKRAD chez les sujets noirs africains n’est pas rare et probablement sous-diagnostiquée.


ABSTRACT
Introduction. Autosomal dominant polycystic kidney disease (APKD) is the most frequent hereditary nephropathy in the world. The aim of our study was to study the epidemiological, clinical, para clinical, therapeutic and evolutionary characteristics of APKD cases in semi-urban areas in Senegal. Materials and methods. This was a prospective, multicenter, transversal study spanning 3 years from 1rst January 2015 to 31th December 2017. Were included in the study, all patients hospitalized or received in consultations, in three nephrology services for an APKD and aged over 15 years. Results. In the 03 years of study, 25 cases of APKD were collected, a prevalence of 0.30%. High blood pressure was present in all our patients. Large palpable kidneys were found in 22 patients (85%) followed by lumbar pain in 21 patients (60%). The majority of patients had renal failure at the diagnosis time. Extra-renal cysts were found in the liver in 8 patients, 68%. The average hemoglobin level was 9.91 g / dl. A genetic study was performed in 2 patients (8%) and confirmed a PKD1 gene mutation. Four patients (16%) had benefited from chronic hemodialysis. Only one patient had received treatment with Vaptan (4%). The evolution was marked by the occurrence of 4 deaths or 16%. Conclusion. APKD in black African subjects is not uncommon and probably underdiagnosed.
