Abstract

RÉSUMÉ
Contexte. Moins du tiers des patients hypertendus traités sont normalisés dans la plupart des pays. Une des causes de l’inefficacité du traitement médical dans l’hypertension artérielle, est souvent son observance insuffisante. Le but de cette étude était d’examiner les facteurs environnementaux susceptibles d’influencer l’observance du traitement médical dans l’Hypertension artérielle. Méthodes. Il s’est agi d’une étude transversale. Le recueil des données a été fait du 1er juin au 30 Novembre 2018, au moyen d’un questionnaire adressé aux patients hypertendus et pris en charge depuis au moins six mois à l’Unité de Cardiologie et au Centre Anti Diabétique d’Abidjan de l’Institut National de Santé Publique. L’observance du traitement a été évalué par le test de GIRERD et al. La qualité de l’environnement a été évaluée avec un questionnaire. Résultats. Nous avons recruté 524 patients dont 190 hommes et 334 femmes, soit un sex ratio H/F de 0,56. L’âge moyen était de 59,1 ± 8,1 ans. 139 patients (26,5%) provenaient d’un environnement favorisé. Trente six patients (6,9%) avaient une bonne observance ; 271 patients (51,7%) avaient de minimes problèmes d’observance et 217 patients (41,1%) avaient une mauvaise observance du traitement. Les patients issus d’un environnement familial favorisé avaient une meilleure observance du traitement par rapport à ceux provenant d’un environnement défavorisé (p=0,000). Conclusion. La qualité de l’environnement familial influence l’observance du traitement et elle est un facteur à prendre en compte dans la prise en charge de l’hypertension artérielle.
ABSTRACT
Inroduction and aim. Less than one-third of treated hypertensive patients are normalized in most countries. One of the causes of ineffective medical treatment for hypertension is insufficient compliance. The purpose of our study was to examine environmental factors that may influence adherence to medical treatment in hypertension. Methods. This was a cross-sectional study. The data were collected from June 1 to November 30, 2018, using a questionnaire sent to hypertensive patients. The patients were treated for at least six months at Cardiology Unit and at the Centre Anti Diabétique d'Abidjan (CADA) of National Institute of Public Health. Adherence to treatment was assessed by the GIRERD et al. test. Quality of the environment was assessed with a questionnaire. Results. We recruited 524 patients including 190 males and 334 females. The mean age was 59.1 ± 8.1 years. 139 patients (26.5%) came from a privileged environment. Thirty six patients (6.9%) had good treatment compliance; 271 patients (51.7%) had minimal compliance problems, and 217 patients (41.1%) had poor treatment compliance. Patients from an privileged family environment had better treatment compliance compared to those from an underprivileged environment (p=0,000).  Conclusion. The quality of the family environment influences treatment adherence and it is a factor to be taken into account in the management of hypertension.
