Abstract

RÉSUMÉ
Introduction. Secrétée par les follicules préantraux et antraux, et sans variation intra-cycle ou inter cycle, l’hormone anti-mullerienne (AMH) est un marqueur fiable de la réserve ovarienne. Son utilisation comme marqueur prédictif de la réponse à la stimulation ovarienne en fait un paramètre important dans la prédictivité des résultats en fécondation in vitro. Objectif. Déterminer une valeur seuil d’AMH permettant de prédire une réponse acceptable à la stimulation ovarienne chez les patientes dont l’âge était compris entre 38 et 42 ans. Méthodologie. Il s’agit d’une étude transversale et analytique avec collecte historico-prospective réalisée de décembre 2016 à juillet 2019 au CHRACERH. Nous avons inclus les patientes avec un AMH détectable selon la technique d’électro chimiluminescence dont l’âge était compris entre 38 et 42 ans et ayant bénéficié d’une stimulation ovarienne en vue d’une fécondation in vitro. Nous avons exclu toute patiente ayant un kyste ovarien en début de stimulation. Le protocole agoniste court était le plus utilisé. Le suivi échographique de la croissance folliculaire était débuté au 6e jour jusqu’à l’atteinte d’un diamètre d’au moins 4 follicules de plus 14 mm pour les 2 ovaires. Le seuil pertinent d’AMH a été déterminé par analyse ROC. Résultats. Nous avons retenu 70 cycles complets dont 67 sous protocole court agoniste et 3 sous protocole long agoniste. Le compte folliculaire moyen était de 6.8 ± 4.1, le nombre moyen de follicules ponctionnés était de 4,13 ± 6,8 et le nombre moyen d’ovocytes matures 5 ± 3, 3 et le nombre moyen d’ovocytes matures obtenus était de 5±3.3. Le taux de mauvaises réponses était de 37,1%. Il y avait une corrélation entre le nombre d’ovocytes matures et l’AMH (r : 0,444 p : 0,00). À l’analyse de courbe ROC, le test AMH restait informatif par rapport à la réponse à la stimulation ovarienne avec une aire sous la courbe (ASC) à 0,745 (p=0.00, IC 95%). La valeur seuil donnée par le point le plus éloigné de la diagonale représentant le test d’apport nul était de 1.1 ng/L, laquelle permettait de prédire une réponse acceptable à la stimulation ovarienne avec une sensibilité de 74,3% et une spécificité de 65,6%. Conclusion. L’AMH est un marqueur prédictif de la réponse à la stimulation ovarienne pour la tranche d’âge étudiée. Le seuil de 1.1ng/l pourrait être utilisée pour décider de l’indication d’une stimulation en vu d’une fécondation in vitro.
ABSTRACT
Introduction. The anti-Mullerian hormone (AMH) is secreted by the pre-antral and antral follicles and incurs little intra-cycle and inter-cycle variation. So, it is a reliable marker of the ovarian reserve. Its use as a predictive marker of the response to ovarian stimulation makes it an important parameter in the predictability of results in vitro fertilization (IVF). Goal. The objective of our work was to determine an AMH threshold value allowing an acceptable response to ovarian stimulation in women aged 38 to 42 years. Methodology. This was a cross-sectional and analytical study with historico prospective collect, carried out from December 2016 to May 2019 at CHRACERH. We included patients with a detectable AMH by electro chimiluminescence technique whose age was comprised between 38 and 42 years. We excluded any patient with an ovarian cyst at the start of stimulation. These patients were on their first attempt at ovarian stimulation for IVF / ICSI (Intra Cytoplasmic Injection of Spermatozoa). The short agonist protocol was most often used (95.7%). Ultrasound monitoring of follicular growth was started at day 6 until at least 4 follicles larger than 14 mm were reached in both ovaries. Results. We selected 70 IVF / ICSI complete cycles including 67 under short agonist protocol and 3 under long agonist protocol. The average follicle count was 6.8 ± 4.1. The average number of retrieved oocytes was 4.13 ± 6.8 and the average number of mature oocytes 5 ± 3.3. The rate of poor responders was 37.1%. There was a positive correlation between the number of mature oocytes and AMH (r: 0.444 p: 0.00). The ROC curve showed that the AMH test remained informative with respect to the response to ovarian stimulation (AUC= 0.745 ; p = 0.00, 95% CI). The threshold value given by the point furthest from the diagonal representing the zero intake test was 1.1 ng / L. This value predicted an acceptable response to ovarian stimulation with a sensitivity of 74.3% and a specificity 65.6%. Conclusion. AMH is a predictive marker of the response to ovarian stimulation for the age group studied. The threshold of 1.1 ng / l can be used to decide on the indication of stimulation in view of in vitro fertilization.
