Abstract

RÉSUMÉ
Nous rapportons le cas d’un patient admis pour empyème sur plaie cranio-cérébrale, compliquée d’un tétanos et formulons des recommandations.
Le patient de 35 ans, a été admis pour troubles de la conscience suite à un trauma crânien. Devant une plaie frontale droite purulente suturée, une contracture des masséters, des crises paroxystiques tétaniques, ainsi qu’un scanner cranio encéphalique montrant un empyème extradural et sous cutané, le diagnostic de tétanos, complication très rare, a été évoqué. Une prise en charge chirurgicale en urgence a été effectuée avec un transfert dans le service d’infectiologie. L’évolution a été favorable après 2 semaines avec des suites simples.

ABSTRACT
We report a patient who survived tetanus as a complication of a craniocerebral wound with empyema. He was a 35 year old male who was admitted in coma, after a head trauma. Later, the wound became infected and he developed trismus, paroxysmal tetanic contractions. Cerebral CT showed epidural empyema. A surgical act was performed. The patient eventually recovered two weeks later.
