Abstract


RÉSUMÉ
Le SARS-CoV-2 est un nouveau coronavirus apparenté au SARS-CoV et au MERS qui est apparu à Wuhan en Décembre 2019. La chauve-souris semble être le réservoir de virus animal. La transmission est essentiellement interhumaine directe via des gouttelettes ou indirecte via des surfaces contaminées, car le virus peut y survivre plusieurs jours. La pathogénie est similaire à celle des autres coronavirus, avec plus fréquemment dans les formes critiques une hypersécrétion de mucus, une dyscoagulabilité et un orage cytokinique qui font la gravité de la maladie. La période d’incubation est de 5 jours en moyenne et le taux de contagiosité est de 2. Les signes cliniques typiques sont infectieux et respiratoires. L’on décrit aussi des formes digestives, neurologiques, cardiaques ou dermatologiques. La maladie peut être asymptomatique.  Les principaux facteurs de risque de mortalité sont l’âge avancé et les comorbidités. Il n’y a pas de traitement spécifique. Mais plusieurs molécules  comme l’hydroxychloroquine ou le remdesivir et des médicaments issus de la pharmacopée traditionnelle africains ont des résultats encourageants. La priorité est la prévention, via un dépistage agressif suivi de prise en charge en environnement protégé), des mesures d’hygiène associées à des gestes barrières rigoureux (hygiène des mains, masques faciaux, distanciation sociale, voire confinement de masse) et une prise en charge multidisciplinaire. Des vaccins sont en cours de développement.
ABSTRACT
SARS-CoV-2 is an emerging human CoV, similar to previous SARS and MERS outbreaks. Bats may be the animal reservoir. Transmission is essentially inter human, through inhalation of infected droplets or contact with contaminated surfaces. Pathogenesis is similar to other CoV, but hyper secretion of mucus, coagulation disorders and cytokine storm are features of critical cases. The mean incubation period is 5.2 days and a R0 of 2.2. Typical clinical signs include association of infectious syndrome and respiratory signs. There may be digestive neurological, cardiac or dermatological manifestations. The risk of death is higher in older people and those with comorbidities (cardiovascular disease, diabetes, hypertension, cancer). There are no specific antiviral drugs. However, association of hydroxychloroquine, azythromycine and other drugs from African pharmacopeia have promising results. Prevention is the cornerstone of control; this includes screening and isolation of positive cases, hygiene measures and multi disciplinary management of severe cases. Vaccines are being developed.
