Abstract

RÉSUMÉ
Le deuil pathologique est une entité clinique connue depuis 1987, date d’apparition du DSM-III-R. Ce diagnostic bien que rare est répété dans le DSM-5, dernière version de la classification américaine, malgré la disparition d’autres affections. Les données cliniques et épidémiologiques dans notre continent sont rares et les critères diagnostiques demeurent complexes et confus pour certains cliniciens au profit de la «dépression mentale». La pandémie à COVID-19 est responsable de nombreuses pertes matérielles, financières et surtout humaines dans le monde en général et dans la population africaine en particulier. Cette population, dotée d’une personnalité sensitive développée, sait s’exprimer devant une perte d’objet. Ce contexte de pandémie mondiale depuis le 11 mars 2020, nous paraît idoine pour revisiter le tableau clinique de ce diagnostic dit secondaire mais invalidant dans le milieu socioprofessionnel. Nous passerons les critères diagnostiques de cette entité selon le DSM-5 américain en revue et nous soulignerons les réalités culturelles de notre continent, très sensible au deuil ainsi qu’au travail de deuil. Notre intérêt étant d’observer longitudinalement cette « affection » comme diagnostic principal plusieurs années après la disparition de cette pandémie, car elle pourrait laisser des blessures psychiques indélébiles.

ABSTRACT
Pathological mourning is a clinical entity known since 1987, when DSM-III-R appeared. This diagnosis, although rare, is conserved in the DSM-5, the latest version of the American classification, despite the disappearance of other conditions. Clinical and epidemiological data in our continent are scarce and diagnostic criteria remain complex and confused for some clinicians in favor of "mental depression". The COVID-19 pandemic is responsible of numerous materials, financial and above all human losses in the world in general and in the African population in particular. This population, endowed with a developed sensitive personality, knows how to express itself in the face of a loss of object. This context of a global pandemic since March 11, 2020, seems appropriate to us to revisit the clinical picture of this so-called secondary diagnosis but invalidating in the socio-professional environment. We will review the diagnostic criteria of this entity according to the American DSM-5 and we will highlight the cultural realities of our continent, very sensitive to mourning as well as to the work of mourning. Our interest being to observe this "condition" longitudinally as the main diagnosis several years after the disappearance of this pandemic, because it could leave indelible psychological wounds.
