Abstract

RÉSUMÉ
Introduction. Les sténoses urétérales bilharziennes étant fréquentes au Mali, le but de notre étude était de déterminer la fréquence, les indications de la réimplantation urétéro-vésicale et d’en évaluer les résultats à l’hôpital de Ségou. Patients et Méthodes. Il s’agissait d’une étude prospective descriptive allant du 1er Juin 2015 au 31 MAI 2016 pour le recrutement des patients et du 1er Juin 2016 au 31 Mai 2018 pour le suivi La technique de réimplantation choisie principalement était celle dite de « trompe d’éléphant ». Résultats. Durant notre étude, nous avons réalisé 28 réimplantations chez 21 patients âgés de 23 à 63 ans avec un âge moyen de 36.7 ans±12,14 ans. Le sexe ratio homme-femme était de 1.6. Les sténoses urétérales d’origine bilharzienne étaient les principales indications de la réimplantation urétéro-vésicale avec une proportion de 90,5% et la technique de réimplantation « trompe d’éléphant » était la plus utilisée. La durée moyenne d’hospitalisation était de 12 Jours ± 2,19. L’évolution était satisfaisante chez 52.4% des patients 03 mois après l’intervention. Les complications étaient à type de suppuration pariétale et de chute prématurée du cathéter urétéral et n’ont concerné que 14,3% des patients. Après 24 mois de suivi une régression de l’urétéro hydronéphrose était observée dans 95,2% des cas à l’échographie. Le contrôle sanguin du taux de créatinine était anormal chez seulement un de nos patients. Conclusion. Au Mali, la sténose du bas uretère d’origine bilharzienne constitue la principale indication de la réimplantation urétéro-vésicale (4, 5,6).


ABSTRACT
Introduction. Bilharzian ureteral stenosis is common in Mali. The aim of our study was to determine the frequency and indications for uretero-vesical reimplantation and to evaluate the results at the hospital in Segou. Patients and methods. This was a prospective descriptive study from June 1, 2015 to May 31, 2016 for patient recruitment and from June 1, 2016 to May 31, 2018 for follow-up. Results. During our study, we performed 28 reimplantations in 21 patients aged 23 to 63 years with a mean age of 36.7±12.14 years. The male-to-female sex ratio was 1.6. Ureteral stenosis of bilharzian origin was the main indication for uretero-vesical reimplantation with a proportion of 90.5% and the "elephant trunk" reimplantation technique was the most used. The average duration of hospitalization was 12 days ± 2.19. The evolution was satisfactory in 52.4% of the patients 03 months after the operation. Complications were parietal suppuration and premature fall of the ureteral catheter and concerned only 14.3% of patients. After 24 months of follow-up, a regression of the uretero-hydronephrosis was observed in 95.2% of cases on ultrasound. Blood monitoring of creatinine levels was abnormal in only one of our patients. Conclusion. In Mali, stenosis of the lower ureter of bilharzian origin constitutes the main indication for uretero-vesical reimplantation
