Abstract

RÉSUMÉ
Introduction. La pyonéphrose est la présence d’une collection purulente dans les cavités rénales dilatées avec destruction partielle ou totale du parenchyme rénal associée à une péri néphrite importante. Notre objectif était d’étudier les aspects épidémiologique, diagnostique et thérapeutique des pyonéphroses dans notre structure. Patients et méthodes. Il s’agissait d’une étude rétrospective descriptive portant sur les cas de pyonéphroses pris en charge dans notre structure entre Janvier 2012 et Décembre 2020. Résultats. Nous avons pris en charge 44 cas de pyonéphroses. L’âge moyen était de 46,26 ± 12 ans. Le sex ratio était de 1,28. Les principaux motifs de consultation ont été la douleur lombo-abdominale, les troubles de la miction et la pyurie avec respectivement 40,91%, 25% et 18,18%. La fièvre (43,18%) et la perception de masse abdomino-lombaire (25%) étaient les signes les plus fréquents à l’examen physique. Les examens d’imagerie ont permis de poser le diagnostic d’une pyonéphrose gauche dans 59,09% (n=26) et droite dans 40,91% (n=18). Les causes de l’obstruction étaient dominées par les lithiases urinaires à 45,45% et la sténose des bas uretères à 38,64% (n= 17). Le traitement a consisté en une néphrectomie dans 93,18% (n=41). Cette néphrectomie a été précédée d’une néphrostomie per cutanée dans 31,82% (n=14). Les bactéries isolées étaient des entérobactéries dominées par Escherichia Coli. Conclusion. La pyonéphrose est une affection grave caractérisée par le retard de diagnostic dans notre contexte. Aucun rein n’a pu être sauvé, le diagnostic étant fait à la phase de fonte purulente du rein.


ABSTRACT
Introduction. Pyonephrosis is the presence of a purulent collection in the dilated renal cavities with partial or total destruction of the renal parenchyma associated with significant periphritis. Our goal was to study the epidemiological, diagnostic and therapeutic aspects of pyonephrosis in our structure. Patients and methods. This was a descriptive retrospective study of pyonephrosis cases managed in our structure between January 2012 and December 2020. Results. We managed 44 cases of pyonephrosis. The median age was 46.26±12 years. The sex ratio was 1.28. The main reasons for consultation were abdominal flank pain, urination disorders and pyuria with 40.91%, 25% and 18.18% respectively. Fever, 43.18% and abdomino-lumbar mass perception, 25% were the most frequent signs on physical examination. Imaging tests diagnosed 59.09% (n=26) left pyonephrosis and 40.91% (n=18) right pyonephrosis. The causes of obstruction were dominated by urinary lithiasis 45.45% and stenosis of the lower ureters 38.64% (n = 17). Treatment consisted of a nephrectomy in 93.18% (n=41). This nephrectomy was preceded by a percutaneous nephrostomy in 31.82% (n=14). The isolated bacteria were enterobacteria dominated by Escherichia Coli. Conclusion. Pyonephrosis is a serious condition characterized by delayed diagnosis in our context. No kidney could be saved the diagnosis being made at the purulent melting phase of the kidney.
