RÉSUMÉ
Introduction. L’incidence de l’infection du site opératoire (ISO) est élevée et varie de 6,8 à 26% en Afrique subsaharienne. Le but de ce travail était d’évaluer l’efficacité d’un dispositif médical, l’anneau rétracteur de plaies (ARP), dans la réduction de ces ISO. Patients et Méthodes. Nous avons mené une étude expérimentale multicentrique dans la ville de Yaoundé. Prospectivement, tous les patients opérés d’un abdomen aigu chirurgical d’origine digestive sur une période de 7 mois étaient colligés et par randomisation simple, répartis en deux groupes : un groupe A correspondant aux patients opérés avec un ARP (SurgiSleeve, Covidien) et un groupe B de patients opérés sans ce dispositif. Les patients étaient suivis jusqu’au 30ème jour postopératoire. La survenue d’une ISO était le principal résultat investigué. Résultats. Au total 128 patients ont été colligés. Leur âge moyen était de 36,9±14,5 ans. L’indication chirurgicale la plus fréquente était la péritonite aigue (39,8%). Trente-huit (29,7%) ont été inclus dans le groupe A, et 90 (71,3%) dans le groupe B. La prévalence globale des ISO était de 35,9%, de 21,1% dans le groupe A et de 42,2% dans le groupe B ; cette différence était statistiquement significative (P=0,023). Le coût global des soins était de 327.000 ± 26.300 FCFA (498,5±40,1 euros) en absence d’infection du site opératoire, contre 501.000 ± 285.000 FCFA (763,7±434,5 euros) en cas de survenue d’une ISO (p=0,04). Conclusion. L’usage de l’ARP dans les urgences chirurgicales digestives réduit de manière statistiquement significative la survenue des ISO et le coût des soins.


ABSTRACT
Background and aim. The incidence of surgical site infection (SSI) is high and ranges from 6.8% to 26% in sub-Saharan Africa. The aim of this study was to evaluate the effectiveness of a medical device, the wound retractor ring, in reducing these SSIs. Patients and Methods. We carried out a multicenter experimental study in the city of Yaoundé. Prospectively, all patients operated on for an acute surgical abdomen of digestive origin over a period of 7 months were collected and by simple randomization, divided into two groups: a group A corresponding to patients operated on with a wound retractor ring (SurgiSleeve, Covidien) and a group B of patients operated without this device. The patients were followed up to the 30th postoperative day. The occurrence of SSI was the main outcome investigated. Results. A total of 128 patients were collected. Their mean age was 36.9±14.5 years. The most common indication for surgery was acute peritonitis (39.8%). Thirty-eight (29.7%) were included in group A, and 90 (71.3%) in group B. The overall prevalence of SSI was 35.9%, 21.1% in group A and 42.2% in group B; this difference was statistically significant (P = 0.023). The overall cost of care was 327,000 ± 26,300 F CFA (498.5 ± 40.1 euros) in the absence of SSI, and 501,000 ± 285,000 F CFA (763.7 ± 434.5 euros) in the event of a SSI (p = 0.04). Conclusion. The use of a wound retractor ring in digestive surgical emergencies reduces the occurrence of SSIs and the cost of care.
