Abstract


RÉSUMÉ
Introduction. Le cancer du testicule est une tumeur maligne rare de l’homme et la plus fréquente de l’adulte jeune âgé de 15 à 35 ans. Le but de notre travail était de présenter leurs aspects cliniques, anatomopathologiques et les limites thérapeutiques. Patients et méthodes. Il s’agissait d’étude rétrospective et descriptive, colligeant les dossiers des patients porteurs d’une grosse bourse, de 2016 à 2020 au service d’urologie. Étaient inclus, les patients âgés de plus de15 ans, porteurs d’une grosse bourse chez qui le bilan clinique, paraclinique et thérapeutique a permis de retenir le diagnostic de cancer du testicule. L’utilisation des photos était faite dans le respect de l’identité du patient et de son consentement. Résultats. Au total, 6 patients atteints de cancer du testicule ont été inclus. La moyenne d’âge était de 28,8 ans. Le motif de consultation était la grosse bourse. Le testicule controlatéral était normal (taille, volume, consistance) et présent au fond de la bourse (n=6). Les marqueurs tumoraux (HCG, LDH, Testostéronémie) étaient élevés chez 3 patients. La TDM (abdominopelvienne et l’Uro-TDM) a mis en évidence des adénopathies (n=2), l’échographie abdominopelvienne était normale. L’analyse histologique de la pièce d’orchidectomie concluait à un carcinome à sac vitellin pur (n=4) et mixte (n=2). Deux patients avaient accepté l’évacuation sanitaire pour une chimiothérapie adjuvante. Conclusion. Le cancer du testicule demeure fréquent chez l’adulte jeune. Le diagnostic de certitude est histologique à partir de la pièce d’orchidectomie haute.


ABSTRACT
Introduction. Testicular cancer is a rare malignant tumour in men and the most common in young adults aged 15-35 years. The aim of our work is to present the clinical aspects, anatomopathological aspects and therapeutic limits. Patients and methods. This was a retrospective and descriptive study, collecting the records of patients with a large bursa, from 2016 to 2020 in the department of urology. Patients over 15 years of age with a large bursa were included, in whom the clinical, paraclinical and therapeutic work-up led to a diagnosis of testicular cancer. The use of the photographs was done with respect to the patient's identity and consent. Results. A total of 6 patients with testicular cancer were included. The average age was 28.8 years. The reason for consultation was the large bursa. The contralateral testis was normal (size, volume, consistency) and present at the bottom of the bursa (n=6). Tumour markers (HCG, LDH, Testosteronemia) were elevated in 3 patients, CT (abdominopelvic and Uro-CT) showed adenopathies (n=2), abdominopelvic ultrasound was normal. Histological analysis of the orchiectomy specimen showed pure (n=4) and mixed (n=2) yolk sac carcinoma. Two patients had accepted adjuvant chemotherapy. Conclusion. Testicular cancer remains frequent in young adults. The diagnosis of certainty is histological from the upper orchidectomy specimen.
