RÉSUMÉ
Introduction. Les complications cardiovasculaires demeurent la première cause de morbi-mortalité chez les patients hémodialysés chronique en dépit de l'amélioration des traitements de suppléance. L’objectif de ce travail était de décrire les signes électriques et échocardiographiques chez les patients hémodialysés chroniques de Dakar. Patients et méthodes. Il s’agit d’une étude transversale descriptive et analytique réalisée dans deux centres d’hémodialyse chronique de Dakar, durant 8 mois. Ont été inclus tous les patients hémodialysés chroniques ayant effectué un ECG et une échographie-Doppler cardiaque. Les paramètres électriques et écho-cardiographiques ont été étudiés. Résultats. Quatre-vingt-dix-sept patients ont été inclus, avec un sex-ratio (H/F) de 0,83. L’âge moyen des patients était de 51,42±15,26 ans. La durée moyenne de dialyse était de 60,38± 30,86 mois. L’électrocardiogramme a objectivé les troubles de repolarisation chez 14 patients, une fibrillation auriculaire chez 2 patients et des troubles de conduction chez 12 patients. Les anomalies écho-cardiographiques étaient notées dans 89,69% des cas. Elles étaient dominées par l’hypertrophie ventriculaire gauche (HVG) chez 50,57%, et les valvulopathies chez 47,12%. L’HVG est associée à une dysfonction systolique dans 13,79% et à une dysfonction diastolique dans 36,78%. Les calcifications valvulaires ont été objectivées dans 28,73% des cas, une dilatation de l’oreillette gauche dans 31,03%, une hypertension artérielle pulmonaire (HTAP) dans 31,03% et une péricardite dans 8,04% des cas. Conclusion. Notre étude a montré que les troubles de repolarisation étaient positivement corrélés au diabète, l’HVG à l’hyperparathyroïdie (p=0,049) et l’HTAP à l’hypoalbuminémie (p=0,045).
ABSTRACT
Background. Cardiovascular complications remain the leading cause of morbidity and mortality in chronic haemodialysis patients despite improvements in replacement therapy. The objective of this study was to describe the electrical and echocardiographic signs in chronic haemodialysis patients in Dakar. Patients and methods. This was a cross sectional descriptive and analytical cross-sectional study carried out in two chronic haemodialysis centres of Dakar for 8 months. All chronic haemodialysis patients for more than 3 months were included and having performed an electrocardiogram and cardiac Doppler ultrasound. Sociodemographic, clinical, biological, dialytic, ecg, and cardiac sonogram parameters were studied. Results. Ninety-seven patients were included, with a sex ratio (M / F) of 0.83. The mean age was 51.42 ± 15.26 years. Nephroangiosclerosis was present in 39.2% and primary glomerulopathies in 20.6%. Mean duration of dialysis was 60.38 ± 30.86 months. ECG showed repolarization abnormalities in 14 patients, atrial fibrillation in 2 patients and conduction abnormalities in 12 patients. Abdnormal echocardiographic findings were present in 89.69% of cases. These were mainly left ventricular hypertrophy (LVH) in 50.57%, and valvular disease in 47.12%. LVH was associated with systolic dysfunction in 13.79% of cases and diastolic dysfunction in 36.78% of cases. Valvular calcifications were present in 28.73% of cases, dilation of the left atrium in 31.03% of cases, pulmonary arterial hypertension (PAH) in 31.03% of cases and pericarditis in 8.04% of cases. Conclusion. Our study showed that repolarization disorders were positively correlated with diabetes (p = 0.01), HVG positively correlated with hyperparathyroidism (p = 0.049) and pulmonary hypertension was positively correlated with hypoalbuminemia (p = 0.045).
