Abstract

RÉSUMÉ
Introduction. Une plaie pénétrante est un traumatisme cervical souvent grave, mal connu, parfois négligé. L’objectif de notre travail était de décrire les aspects épidémio-cliniques et thérapeutiques des plaies pénétrantes du cou en pratique civile par arme blanche dans le service d’ORL- CCF du Centre Hospitalier National Universitaire (CHNU) de Fann. Matériels et méthodes. Il s’agit d’une étude rétrospective menée au service d’ORL et CCF du CHNU de Fann durant la période du 1er Janvier 2014 au 31 décembre 2019. Résultats. Au total,122 dossiers ont été colligés. Il s’agissait de 117 hommes et 5 femmes soit un sex ratio de 23, 4. L’âge moyen des patients était de 27, 25 ans avec des extrêmes allant de 2 ans à 64 ans. La circonstance de survenue était dans 42,6% des cas une rixe et dans 21,3% une agression. L’agent étiologique le plus souvent rencontré était le couteau dans 46,7%. On retrouvait parmi les 52 lésions vasculaires, 18 plaies de la veine jugulaire interne, une lésion carotidienne commune. L’axe viscéral était lésé dans 28 cas. Les suites opératoires étaient marquées par un cas d’hématome, un cas de lymphorrhée, un cas de médiastinite et une difficulté de décanulation chez deux patients. Quatre (4) cas de décès ont été rapportés. Conclusion. Les plaies pénétrantes du cou demeurent une urgence et une pathologie d’actualité dont l’amélioration de la prise en charge passe par une consultation précoce et le relèvement du plateau technique.
ABSTRACT
Introduction. Penetrating neck is an often severe cervical trauma, poorly known and sometimes neglected. The objective of our work was to describe the epidemiological, clinical and therapeutic aspects of penetrating neck stab wounds in civilian practice at the ENT and Head and Neck department of Fann Teaching Hospital. Materials and methods. This was a cross sectional retrospective study conducted at the ENT and Head and Neck department of Fann Teaching Hospital during the period from January 1st, 2014 to December 31st, 2019. Results. A total of 122 files were collected. There were 117 males and 5 females, for a sex ratio of 23.4. The mean age of the patients was 27.25 years with extremes ranging from 2 years to 64 years. The circumstance of occurrence was in 42.6% of the cases a fight and in 21.3% an assault. The etiological agent most often encountered was the knife in 46.7% of cases. Among the 52 vascular lesions, 18 wounds of the internal jugular vein, one common carotid injury was found. The visceral axis was injured in 28 cases. The postoperative outcome was marked by one case of hematoma, one case of lymphorrhea, one case of mediastinitis, and difficulty in decanulation in two patients. Four (4) cases of death were reported. Conclusion. Penetrating neck wounds remain an emergency and a current pathology whose management can only be improved through early consultation and by improving the technical means.
