Abstract

RÉSUMÉ
Introduction. L’améloblastome mandibulaire est une tumeur bénigne odontogène caractérisée par son agressivité locale. L’objectif de cette étude était de faire l’état des lieux de l’améloblastome dans le service de stomatologie et chirurgie maxillofaciale du Centre Hospitalier et Universitaire de Bouaké afin d’améliorer la prise en charge de cette pathologie. Patients et méthodes. Il s’agit d’une étude de cohorte descriptive à collecte rétrospective des cas d’améloblastomes mandibulaires et pris en charge dans le service de stomatologie et chirurgie maxillofaciale pendant la période de 04 ans allant du 1er Janvier 2017 au 31 Décembre 2020. Résultats. Notre échantillon comptait 10 patients dont l’âge moyen était de 35,8 ans avec des extrêmes de 18 et 53 ans, et un sex-ratio de 1,5. La tuméfaction mandibulaire était le motif de consultation le plus retrouvé (100%). Le délai moyen de consultation était de 4,8 ans avec des extrêmes de 6 mois et 10 ans. La tomodensitométrie maxillofaciale était l’examen clé du diagnostic. L’aspect polygéodique a été retrouvé chez 8 patients (80%). L’examen anatomopathologique a révélé une prédominance du type folliculaire (5 cas). Le traitement chirurgical a été radical dans tous les cas. L’évolution a été favorable sans aucune récidive chez tous les patients. Conclusion. L’améloblastome de la mandibule a des répercussions esthétiques et fonctionnelles significative. Les adultes jeunes sont les plus touchés. Ils consultent à un stade avancé de la maladie donnant des tumeurs géantes dites historiques. La résection chirurgicale reste le traitement de choix. Une prise en charge adaptée permet de réduire le taux de récidive.
ABSTRACT
Introduction. Mandibular ameloblastoma is a benign odontogenic tumor characterized by its local aggressiveness. The objective of this study was to describe the clinical features and the management of ameloblastoma in the department of stomatology and maxillofacial surgery of the university teaching hospital of Bouake. Patients and methods. This was a descriptive cohort study with retrospective collection of cases of mandibular ameloblastomas and treated in the department of stomatology and maxillofacial surgery during the 04-year period from January 1, 2017 to December 31, 2020. Results. Our sample included 10 patients whose average age was 35.8 years with extremes of 18 and 53 years; the sex ratio was 1.5. Mandibular tumefaction was the most common reason for consultation (100%). The mean time delay to consultation was 4.8 years with extremes of 6 months and 10 years. Maxillofacial computed tomography was the key diagnostic test. The polygeodic presentation was found in 8 patients (80%). Histological examination revealed predominance of the follicular type (5 cases). Surgical treatment was radical in all cases. The evolution was favorable without any recurrence in all patients. Conclusion. Mandibular ameloblastoma may cause serious esthetic and functional prejudice of the face. It affects mainly young male adults. Most patients are seen at an advanced stage leading to reconstruction problems. Surgical resection is the best modality of treatment. Proper management of reduces significantly the risk of recurrence.
