Abstract

RÉSUMÉ
OBJECTIFS. Évaluer le rôle de l’hypomotilité antrale comme facteur étiologique de la dyspepsie fonctionnelle chez le sujet noir camerounais.
MÉTHODOLOGIE. Il s’agit d’un essai clinique non randomisé qui a inclus 25 sujets dyspeptiques et 16 témoins sains. Tous avaient eu une fibroscopie gastrique négative pour une lésion organique. Le jour du test, nous avons fait une échographie abdominale basale à jeun normale avec mesure de la surface antrale. Puis les sujets ont ingéré un repas semi liquide de 609 calories et nous avons mesuré par échographie la fréquence des contractions antrales et la surface antrale toutes les 15 mn pendant 150 mn. Nous avons ensuite calculé le taux de vidange gastrique et l’index de motilité antrale et comparé les valus dans les deux groupes
RÉSULTATS. La surface antrale moyenne à jeun était comparable dans les deux groupes. Le temps de demi vidange moyen était de 150 mn chez les dyspeptiques contre 127 mn chez les témoins. Le taux de vidange gastrique moyen de la 15é mn des dyspeptiques était plus élevé par rapport aux contrôles (21 % contre 14%) et la tendance a été maintenue jusqu’à la fin. Le nombre moyen de contractions pour 3 mn des dyspeptiques était de 9.2 contre 8.8 chez les contrôles. L’amplitude antrale moyenne était plus élevée chez les dyspeptiques par rapport aux témoins (43% vs 30%)). L’index de motilité moyen des dyspeptiques était plus élevé par rapport aux contrôles (4,2 contre 3,3). Toutefois aucune de ces différences n’était statistiquement significative
CONCLUSION. Bien que les tests statistiques ne trouvent pas de différence significative, les sujets camerounais souffrant de dyspepsie fonctionnelle pourraient avoir une vidange gastrique ralentie, mais une élévation du taux de vidange, de l’amplitude de contraction antrale et de l’index de motilité antrale.

ABSTRACT
AIM: To evaluate the role of low gastric emptying (gastroparesis) as an etiological factor of functional dyspepsia in black Cameroonian patients
METHODS. This was a non randomized clinical trial that included 25 patients with functional dyspepsia and 16 controls without dyspepsia. Fibroscopy was normal in both groups as well as basal abdominal ultrasound. With ultrasound, we measured the fasting antral surface the, both groups ingested a semi liquid meal of 609 calories. We then measured every 15 mn during 150 mn, the frequency of antral contractions and the antral surface. We finally compared the calculated gastric emptying rate and antral motility index between the 2 groups.
RESULTS. Fasting antral surface was similar in both groups. After the meal, antral half emptying time was longer in the dyspeptic group but the empting rate was higher. The frequency of antral contractions was nearly the same, but the antral amplitude was higher in the dyspeptic group as well as the antral motility index. But no difference was statistically significant.
CONCLUSION. Although the differences are not statistically significant, Cameroonian patients with functional dyspepsia may be inclined to have slow gastric emptying (gastroparesis), but increased antral emptying rate, antral contraction amplitude and motility index
