Abstract

ABSTRACT
BACKGROUND AND OBJECTIVE. Heart rate variability (HRV) has emerged in the West as an important prognostic factor in congestive heart failure (CHF), especially for risk stratification of sudden death. The objective of this study was to analyze the HRV in a sample of patients with CHF and deduce the direction of variation.
METHODS. This was a descriptive study in patients with CHF. Patients with atrial fibrillation were excluded. Holter ECG recording (Recorder KONTRON MEDICAL INSTRUMENTS) was performed in 30 subjects with CHF (aged 60 ± 16 years, NYHA class II to IV, EF: 34 ± 6.7%) and 10 normal subjects matched by sex and age groups. The calculation of the parameters of HRV was done with the software: "Advanced HRV Analysis Software ®" which provides the conventional parameters of temporal analysis (SDNN), the frequency analysis (LF, HF, LF / HF) the parameters of the Poincaré plot (SD1, SD2).
RESULTS. All the HRV parameters were abnormally low in subjects with CHF compared to controls with a statistically significant difference: p <0.0001 for SDNN, p <0.0001 for LF and HF, p <0.0001 for the LF / HF ratio, p <0.0001 for SD1, p <0.05 for SD2.Altered heart rate variability was correlated with CHF, higher NYHA class, reduced ejection fraction and with the presence of hypertension and diabetes mellitus. The alteration of the HRV appeared independent of the etiology of CHF and the type of myocardial dysfunction.
CONCLUSION. This work shows similar results to those found by most studies in the literature.

 

RÉSUMÉ
OBJECTIFS. La variabilité sinusale (VRS) s’est imposée en occident comme un important facteur pronostique dans l’insuffisance cardiaque congestive (ICC), en particulier pour la stratification du risque de mort subite. L’objectif de ce travail était d’analyser la VRS sur un échantillon de patients en ICC et en déduire le sens de variation.
MÉTHODES. Il s’agit d’une étude descriptive portant sur des patients en ICC. Les cas de fibrillation auriculaire ont été exclus. Un enregistrement Holter de l’électrocardiogramme (ECG) (Enregistreur KONTRON MEDICAL INSTRUMENTS) a été effectué chez 30 sujets en ICC (âgés de 60±16 ans, classe NYHA II à IV, FE : 34±6,7%) et chez 10 sujets normaux appariés par sexe et par tranches d’âges. Le calcul des paramètres de la VRS a été fait avec le logiciel : «Advanced HRV Analysis Software®» qui fournit les paramètres classiques de l’analyse temporelle (SDNN), de l’analyse fréquentielle (LF, HF, LF/HF) et les paramètres du graphe de Poincaré (SD1, SD2).
RÉSULTATS. Tous les paramètres de VRS étaient abaissés chez les sujets en ICC par rapport aux sujets témoins avec une différence statistiquement significative: p<0,0001 pour SDNN, p<0,0001 pour LF et HF, p=0,0001 pour le ratio LF/HF, p<0,0001 pour SD1, p<0,05 pour SD2. Chez les patients en ICC, l’altération de la VRS était d’autant plus importante que la classe NYHA était avancée, que la fraction d’éjection systolique était effondrée et en cas d’association d’un diabète ou d’une hypertension artérielle. L’altération de la VRS apparaissait indépendante de l’étiologie de l’IC et du type de dysfonction myocardique.
CONCLUSION. Ce travail montre des résultats similaires à ceux trouvés par la plupart des études dans la littérature.
