Abstract

RÉSUMÉ
Introduction. Les accidents de la circulation routière (ACR) représentent une cause prépondérante de fractures faciales dans les contextes à ressources limitées. Cette étude visait à caractériser les profils épidémiologiques et anatomocliniques des fractures faciales post-ACR au CHU Sourô Sanou de Bobo-Dioulasso. Matériel et méthodes. Une étude transversale rétrospective a été menée du 1ᵉʳ janvier 2021 au 31 décembre 2023 incluant tous les patients présentant des fractures faciales radiologiquement confirmées (tomodensitométrie, radiographies de Blondeau/Hirtz, orthopantomogramme) consécutives à un ACR. Les données épidémiologiques, mécanismes lésionnels et lésions associées ont été analysées via Epi Info 7.2.5.0. Résultats. Parmi 340 cas inclus, une prédominance masculine (91%) et jeune (35% âgés de 20-30 ans) a été observée. Les deux-roues motorisés étaient impliqués dans 87% des accidents, avec des collisions inter-deux-roues dans 51,8% des cas. Les fractures zygomatiques constituaient la lésion faciale la plus fréquente (32,57%). Les traumatismes associés incluaient principalement des lésions crânio-encéphaliques (57,98%) et des fractures des membres (28,58%). Conclusion. Cette étude identifie les jeunes conducteurs masculins de deux-roues comme population la plus vulnérable aux fractures faciales post-ACR au Burkina Faso. Elle plaide pour un renforcement immédiat des politiques de sécurité routière et le développement urgent de transports collectifs sécurisés à Bobo-Dioulasso.
ABSTRACT
Introduction. Road traffic accidents (RTAs) constitute a leading cause of facial fractures in resource-limited settings. This study aimed to characterize the epidemiological and clinico-anatomical profiles of post-RTA facial fractures at Sourô Sanou University Hospital in Bobo-Dioulasso. Material and methods. A retrospective cross-sectional study was conducted from January 1, 2021, to December 31, 2023, including all patients with radiologically confirmed facial fractures (CT scan, Blondeau/Hirtz radiographs, orthopantomogram) secondary to RTAs. Epidemiological data, injury mechanisms, and associated lesions were analyzed using Epi Info 7.2.5.0. Results. Among 340 included cases, a male (91%) and young (35% aged 20-30 years) predominance was observed. Motorized two-wheelers were involved in 87% of accidents, with inter-two-wheeler collisions accounting for 51.8%. Zygomatic fractures were the most frequent facial injury (32.57%). Associated traumas primarily included craniocerebral injuries (57.98%) and limb fractures (28.58%). Conclusion. This study identifies young male two-wheeler riders as the most vulnerable group to post-RTA facial fractures in Burkina Faso. It advocates for immediate road safety policy reinforcement and urgent development of secure public transport in Bobo-Dioulasso.
