Abstract
ABSTRACT
Introduction. Post-thoracotomy pain management remains a major clinical challenge, particularly in resource-constrained settings. While regional analgesia is a cornerstone of recovery, the optimal modalities for peripheral local anesthetic delivery are still debated. This study aimed to evaluate the efficacy of intraoperative intercostal bupivacaine infiltration on opioid consumption and pain after thoracotomy for rib osteosynthesis in a university hospital in Cameroon. Methodology. We conducted a prospective, randomized, double-blind, placebo-controlled crossover trial involving 17 adult patients undergoing rib osteosynthesis via posterolateral thoracotomy. At closure, a 20 ml bolus of 0.25% bupivacaine or saline was infiltrated into the intercostal spaces above and below the incision. Standardized multimodal analgesia was administered postoperatively. The primary outcome was 24-hour morphine consumption measured via a patient-request system. Pain was assessed using the visual analog scale (VAS) at H1, H6, H24, and H48. Results. The mean 24-hour morphine consumption was significantly lower with bupivacaine (29 ± 16 mg) than with placebo (44 ± 21 mg; p=0.04). VAS scores were also significantly reduced in the bupivacaine group at all assessment points. No adverse events related to the technique were observed. Conclusion. Intraoperative intercostal bupivacaine infiltration significantly reduces morphine consumption and pain intensity after thoracotomy for chest trauma. This simple, safe, and low-cost technique constitutes a primary analgesic strategy, particularly well-suited to resource-constrained care contexts like Sub-Saharan Africa, with potential implications for recovery and health economics.
RÉSUMÉ
Introduction. Conclusion.
