Abstract
RÉSUMÉ
Introduction. La chimiothérapie à base de cisplatine, pierre angulaire du traitement de nombreux cancers, expose à une néphrotoxicité dose-dépendante dont l'insuffisance rénale aiguë (IRA) est la complication la plus redoutable. L'hyperhydratation, méthode standard de prévention, réduit l'incidence de l'IRA sans la supprimer, justifiant la recherche de stratégies adjuvantes. Cette étude visait à évaluer l'efficacité du sulfate de magnésium associé à l'hyperhydratation dans la prévention de l'IRA induite par le cisplatine à l'Institut de Cancérologie d'Akanda (ICA). Méthodes. Nous avons mené une étude prospective, contrôlée et ouverte, de novembre 2023 à avril 2024, incluant des patients ≥18 ans recevant du cisplatine (clairance ≥60 mL/min, performance status 0-2). Les patients ont été randomisés en deux bras : hyperhydratation + sulfate de magnésium (bras A) versus hyperhydratation seule (bras B). La créatininémie a été mesurée avant et après chaque cure. L'IRA a été définie par une élévation de la créatinine ≥1,5 fois la valeur basale ou une diurèse <500 mL/24h. Résultats. Quatorze patients ont été inclus (âge moyen 47,1 ans ; sex-ratio 1,8). La créatinine basale moyenne était de 67,6 µmol/L (clairance 101,1 mL/min). Une IRA est survenue chez 35,7% des patients : 12,5% (1/8) dans le bras A contre 66,7% (4/6) dans le bras B (OR=0,071 ; IC95% 0,005-1,059 ; p=0,063). Aucune différence significative n'a été observée selon le sexe, l'âge ou la dose de cisplatine. Conclusion. L'association de sulfate de magnésium à l'hyperhydratation semble réduire l'incidence de l'IRA après chimiothérapie par cisplatine, sans atteindre la significativité statistique, probablement en raison de la taille limitée de l'échantillon. Des études à plus grande échelle sont nécessaires pour confirmer cet effet néphroprotecteur.
ABSTRACT
Introduction. Cisplatin-based chemotherapy, a cornerstone of many cancer treatments, exposes patients to dose-dependent nephrotoxicity, of which acute kidney injury (AKI) is the most dreaded complication. Hydration, the standard preventive measure, reduces AKI incidence but does not eliminate it, justifying the search for adjuvant strategies. This study aimed to evaluate the efficacy of magnesium sulfate combined with hydration in preventing cisplatin-induced AKI at the Akanda Cancer Institute (ICA). Methods. We conducted a prospective, controlled, open-label study from November 2023 to April 2024, including patients ≥18 years receiving cisplatin (clearance ≥60 mL/min, performance status 0-2). Patients were randomized into two arms: hydration + magnesium sulfate (arm A) versus hydration alone (arm B). Serum creatinine was measured before and after each cycle. AKI was defined as a creatinine increase ≥1.5 times baseline or urine output <500 mL/24h. Results. Fourteen patients were included (mean age 47.1 years; sex ratio 1.8). Mean baseline creatinine was 67.6 µmol/L (clearance 101.1 mL/min). AKI occurred in 35.7% of patients: 12.5% (1/8) in arm A versus 66.7% (4/6) in arm B (OR=0.071; 95%CI 0.005-1.059; p=0.063). No significant difference was observed according to sex, age, or cisplatin dose. Conclusion. The addition of magnesium sulfate to hydration appears to reduce AKI incidence after cisplatin chemotherapy, without reaching statistical significance, likely due to the small sample size. Larger studies are needed to confirm this nephroprotective effect.
