Abstract
RÉSUMÉ
Introduction. Le glaucome, neuropathie optique dégénérative, représente la deuxième cause mondiale de cécité irréversible. Sa détection précoce, cruciale pour préserver la fonction visuelle, repose en grande partie sur la compétence des médecins de premier recours, particulièrement dans les pays où l'accès aux ophtalmologistes est limité. Au Niger, où les soins ophtalmologiques sont centralisés, les médecins généralistes sont en première ligne. Cette étude visait à évaluer les connaissances, attitudes et pratiques des médecins non ophtalmologistes du Niger face au glaucome. Méthodes. Nous avons mené une étude transversale descriptive du 10 août au 10 octobre 2025 auprès de médecins exerçant dans les structures sanitaires nigériennes. Les données ont été recueillies via un questionnaire structuré, diffusé en ligne et sous format papier, puis analysées avec les logiciels Excel et SPSS. Résultats. Nous avons inclus 260 médecins (âge moyen 38,2 ans ; sex-ratio 2,1). Si 83,8% savaient définir le glaucome comme une neuropathie optique chronique, seuls 39,6% identifiaient correctement les antécédents familiaux comme principal facteur de risque. La douleur oculaire intense (45%) et les céphalées (27%) étaient les signes d'alerte les plus cités. La mesure de la pression intraoculaire était considérée comme l'examen clé par 63,1% des participants, alors que le fond d'œil (21,2%) et le champ visuel (7,3%) étaient moins évoqués. Si 85% reconnaissaient le caractère héréditaire du glaucome et 80% orientaient systématiquement les patients à risque, 37,3% des répondants citaient un manque de connaissances comme obstacle majeur au dépistage. Conclusion. Les médecins nigériens non ophtalmologistes possèdent des connaissances théoriques satisfaisantes sur le glaucome mais présentent des lacunes pratiques significatives. Le renforcement de la formation continue et l'amélioration de l'accès aux outils diagnostiques sont des priorités pour améliorer le dépistage précoce.
ABSTRACT
Introduction. In Niger, data on practitioners’ competence in managing glaucoma remain limited. This study aimed to assess the knowledge, attitudes and practices of doctors practising in Niger with regard to glaucoma. Methods. This was a descriptive cross-sectional study conducted from 10 August to 10 October 2025 amongst doctors practising in healthcare facilities in Niger. Data were collected via a structured questionnaire distributed online and in paper format. Results. The study included 260 participants, aged between 21 and 61 years, with the majority in the 30–39 age group (63.8 per cent). Respondents practised mainly in public hospitals (45.4 per cent) and in the Niamey region (63.9 per cent). 39.6 per cent of participants had between 6 and 10 years’ professional experience, and 58.0 per cent had received training in ophthalmology. 83.8 per cent of doctors were aware of the definition of glaucoma as chronic optic neuropathy. Family history was the most frequently cited risk factor (39.6 per cent). Severe eye pain was the most commonly identified warning sign (45.0 per cent). Intraocular pressure was considered the key test by 63.1 per cent of participants. The hereditary nature of glaucoma was recognised by 85.0 per cent of respondents, and treatment with eye drops was preferred by 60.4 per cent of them. 80.0 per cent of doctors routinely referred at-risk patients to an ophthalmologist. Conclusion. Doctors in Niger have a comprehensive understanding of glaucoma and refer at-risk patients to specialist centres. However, early screening remains limited by a lack of specific training, awareness and access to diagnostic equipment.
