Abstract

RÉSUMÉ
Introduction. L'hypertension rénovasculaire (HRV), secondaire à une sténose de l'artère rénale (SAR), constitue la cause la plus fréquente d'hypertension artérielle secondaire potentiellement curable. La scintigraphie rénale au captopril est un examen fonctionnel non invasif qui permet d'évaluer l'imputabilité hémodynamique d'une sténose dans la genèse de l'HTA. Cette étude avait pour but d'évaluer l'apport de la scintigraphie rénale au 99mTc-MAG3 avec test au captopril dans le diagnostic de l'HRV et d'illustrer son utilité clinique à travers une série de cas. Méthodes. Nous avons mené une étude prospective descriptive de mars à août 2024 à l'Institut des Radio-Isotopes de Niamey, incluant trois patients hypertendus suspects d'HRV. Chaque patient a bénéficié d'une scintigraphie rénale dynamique au 99mTc-MAG3 à l'état basal et une heure après administration orale de 50 mg de captopril. Les paramètres analysés étaient le temps de pic d'activité (Tmax) et la perfusion rénale relative. Résultats. L'âge moyen des patients était de 39,3 ans (extrêmes : 31-47 ans). L'échodoppler avait objectivé une sténose unilatérale de l'artère rénale droite chez les trois patients. Chez deux patients (A et B), la scintigraphie au captopril a montré des modifications significatives : augmentation du Tmax (>2 minutes) et chute de la perfusion relative (>5%) du rein droit, confirmant la responsabilité hémodynamique de la sténose. Chez le troisième patient (C), aucune altération fonctionnelle significative n'a été observée, suggérant une sténose non hémodynamiquement significative. Conclusion. La scintigraphie rénale au captopril est un outil diagnostique utile pour identifier les sténoses rénales hémodynamiquement significatives chez les patients hypertendus suspects d'HRV. Elle permet de sélectionner les patients candidats à une revascularisation, particulièrement dans les contextes où l'accès à l'imagerie morphologique de confirmation est limité. Des études avec des effectifs plus importants sont nécessaires pour valider ces observations.


ABSTRACT
Introduction. Renovascular hypertension (RVH), secondary to renal artery stenosis (RAS), is the most common cause of potentially curable secondary hypertension. Captopril renal scintigraphy is a non-invasive functional test that assesses the hemodynamic significance of a stenosis in the genesis of hypertension. This study aimed to evaluate the contribution of 99mTc-MAG3 renal scintigraphy with captopril test in the diagnosis of RVH and to illustrate its clinical utility through a case series. Methods. We conducted a prospective descriptive study from March to August 2024 at the Institute of Radioisotopes in Niamey, including three hypertensive patients suspected of RVH. Each patient underwent dynamic 99mTc-MAG3 renal scintigraphy at baseline and one hour after oral administration of 50 mg captopril. The parameters analyzed were time to peak activity (Tmax) and relative renal perfusion. Results. The mean age of patients was 39.3 years (range: 31-47 years). Doppler ultrasound had shown unilateral right renal artery stenosis in all three patients. In two patients (A and B), captopril scintigraphy showed significant changes: increased Tmax (>2 minutes) and decreased relative perfusion (>5%) of the right kidney, confirming the hemodynamic significance of the stenosis. In the third patient (C), no significant functional alteration was observed, suggesting a non-hemodynamically significant stenosis. Conclusion. Captopril renal scintigraphy is a useful diagnostic tool for identifying hemodynamically significant renal stenoses in hypertensive patients suspected of RVH. It helps select patients eligible for revascularization, particularly in settings where access to confirmatory morphological imaging is limited. Larger studies are needed to validate these findings.
