Abstract


RÉSUMÉ
Introduction. Les fractures mandibulaires représentent 46,67 à 86,62% des fractures du complexe maxillo-facial. Leur étiologie varie d’une région du monde à l’autre. La clinique est diversifiée et la prise en charge bien codifiée. L’objectif de cette étude était de décrire les aspects épidémiologiques, cliniques des fractures mandibulaires traitées par procédé orthopédique. Méthodologie. Nous avons mené une étude rétrospective dans le service d’odontostomatologie de l’Hôpital de District de Kumba, sur une période allant de Janvier 2012 à Décembre 2014. Était inclus tout dossier de patient traité chez qui le diagnostic de fracture mandibulaire avait été posé cliniquement et confirmé par les examens radiographiques. Résultats. 47 cas ont été colligés dont 36 hommes (76,60%) et 11 femmes (23,40%). Le pic de fréquence était situé entre 21-30 ans et l’âge moyen était de 28,21 ans. L’étiologie dominante était les accidents de la voie publique, suivie des chutes. Sur le plan anatomique, les 57 fractures recensées étaient localisées préférentiellement sur la branche horizontale et la symphyse. Le blocage intermaxillaire par la ligature d’Ivy a été le traitement orthopédique le plus utilisé. Le taux de complication était de 19,14%. Conclusion. Les fractures mandibulaires sont des pathologies rencontrées dans le Sud Ouest du Cameroun. Les résultats thérapeutiques satisfaisants associés au taux de complication relativement bas dans notre série nous font penser que le traitement orthopédique est une bonne option dans cette partie du pays.

ABSTRACT
Introduction. Mandible fractures constitute a high proportion of maxillo-facial fractures. Their etiology varies from one region to another of the world. Clinical presentation is varied and treatment modalities are well established. The aim of our study was to describe epidemiological and clinical profiles of mandible fractures treated by orthopedic process. Methodology. We carried out a descriptive cross sectional retrospective study at the Odonto-stomatology unit of Kumba District Hospital between January 2012 and December 2015. We recruited subjects with a clinical diagnosis of mandible fracture and confirmed by the radiological exam. Results. We studied 47 patients, among whom there were 36 males (76.6%) and 11 females (23.4%). The mean age was 28.21 years and most of cases were found between 21-30 years. Road traffic accidents were the main etiology followed by falls. 57 fractures were found and these lesions were localized mainly at the body and symphysis of the mandible. Intermaxillary fixation using Ivy ligature was the commonest orthopedic treatment. Our complication rate was 19.14%. Conclusion. Mandible fractures are encountered in THE South West region of Cameroon. There are mainly due to road traffic accidents. Satisfactory results are obtained by orthopedic treatment.
