La pré-éclampsie (PE) ou toxémie gravidique, est une pathologie maternelle réalisant une hypo-perfusion utéro-placentaire, responsable d’une morbi-mortalité maternelle élevée. L’objectif de notre travail était de déterminer la relation entre pré-éclampsie et pronostic du nouveau-né. Méthodologie. Il s’agit d’une étude prospective,  comparative réalisée au centre de santé de référence de la Commune V du district de Bamako. Deux groupes de 200 nouveau-nés chacun ont été constitués. Le groupe « Cas » était constitué de nouveau-nés issus de grossesses compliquées de pré-éclampsie, et le groupe « Témoin » ceux issus de grossesses normales. Les variables étudiées étaient les caractéristiques maternelles, fœtales et les suites néonatales. L’analyse statistique a été faite avec SPSS 20.0 avec les tests de Khi-2, Student, Man Whitney et la régression logistique avec un risque alpha à 5%. Résultats. Seuls 77 nouveau-nés du groupe « Cas » ont pu être mis immédiatement au sein maternel. Des différences significatives ont été observées entre les deux groupes sur les mesures anthropométriques et les données de morbidité fœtale. En analyse multivariée il existait une association statistiquement significative entre pré-éclampsie et le risque de prématurité, d’hypotrophie fœtale. Les motifs de réanimation néonatale étaient dominés par la prématurité, l’asphyxie néonatale et l’hypotrophie. Conclusion. La pré-éclampsie est génératrice de complications fœtales comme le retard de croissance intra-utérin, la prématurité et l’hypotrophie fœtale, avec un recours plus important aux soins de réanimation néonatale.<br /><br />ABSTRACT<br />Aim. Pre-eclampsia is a maternal pathology which causes a chronic deficiency in utero-placental perfusion with major fetal consquencee. The aim of our study was to determine the neonatal outcomes of preeclampsia in Bamako. Methods. This was a prospective, analytic and comparative study performed in the Commune V Hospital in Bamako. Four hundred newborns were included and divided in two groups. The « Case » group was constituted of newborns from mothers who have developed a pre-eclampsia, and the « Control » group was made of newborns of normal pregnancies. Our study variables were the fetomaternal clinical characteristics, and fetal outcomes. Statistical analysis was performed with SPSS 20.0 using Khi-2, Student, Man Whitney and logistic regression for an alpha error of 5%. Results. Only 77 newborns over 200 of the « Case » group were fit for immediate breastfeeding. Statistical differences were found between « Case » and « control » groups regarding newborn anthropometric measurements, and neonatal morbidity. Multivarious analysis showed a statistical relationship between pre-eclampsia and complications such as prematurity, fetal hypotrophy and need for neonatal intensive care unit admissions (NICU). The most common reasons for NICU admissions were prematurity, neonatal asphyxia and hypotrophy. Conclusion. Pre-eclampsia is a leading cause of multiple fetal harms like intrauterine fetal growth retardation, prematurity and hypotrophy.
