Ils se présentent comme une lésion expansive, quelque fois asymptomatique. Leur localisation au niveau du SNC reste rare ; mais se développent préférentiellement dans le parenchyme cérébral. Nous rapportons un cas de tuberculome épidural intrarachidien chez une patiente de 11 ans admise pour déficit moteur des membres inférieurs. L’interrogatoire retrouvait une claudication médullaire avec réduction du périmètre de la marche suivie d’une impotence totale en moins de 6 mois et des troubles génitaux sphinctériens. L’examen neurologique montrait une paraplégie spastique prédominant aux membres inferieurs avec une vivacité des reflexes ostéo-tendineux, une abolition des réflexes cutané abdominaux aux étages moyens et inférieurs, un signe de Babinski bilatéral ; une hypoesthésie à niveau T7 et un syndrome rachidien thoracique. L’IRM médullaire était en faveur d’un processus intradural extra médullaire de type méningiome responsable d’une compression médullaire postérieure de T7 – T9. La radiographie du thorax était normale et l’intradermoréaction à la tuberculine négative. La biologie révélait une anémie microcytaire hypochrome, une CRP à 48 mg/L. Elle a été mise sous bolus de corticoïdes et transférée en neurochirurgie pour prise en charge. L’anatomopathologie de la pièce opératoire après exérèse chirurgicale a conclu à un granulome tuberculeux caséo-folliculaire. Elle a alors été mise sous antituberculeux.<br />ABSTRACT<br />Tuberculomas are masses of granulomatous tissue with content and limited by the host’s immune system. They present as an expansive lesion, sometimes asymptomatic. Their localization at the CNS remains rare; but preferentially develop in the brain parenchyma. We report a case of intraspinal epidural tuberculoma in an 11-year-old patient admitted for motor deficit in the lower limbs. Interrogation found spinal cord claudication with reduced gait circumference followed by total impotence in less than 6 months and genital sphincter disorders. Neurological examination showed predominantly spastic paraplegia in the lower limbs with briskness of the osteotendinous reflexes, abolition of abdominal skin reflexes in the middle and lower floors, a bilateral Babinski sign; T7 hypoaesthesia and thoracic spinal syndrome. The spinal cord MRI supported a meningioma-like extra-medullary intradural process responsible for posterior spinal cord compression of T7 - T9. The chest x-ray was normal and the tuberculin skin reaction negative. Biology revealed microcytic hypochromic anemia, CRP at 48 mg / L. She was put on a corticosteroid bolus and transferred to neurosurgery for management. The anatomopathology of the operative specimen after surgical excision concluded in a caseo-follicular tuberculous granuloma.
