Le but de cet article était de présenter les difficultés du diagnostic de malignité d’un phéochromocytome dans un contexte aux ressources matérielles limitées .Il s’agissait d’une patiente de 49 ans souffrant d’une lombalgie depuis 10 ans et traitée pour hypertension artérielle et diabète de type II .Le Scanner a montré une masse surrénalienne droite et la biologie une élévation de la glycémie et des métanephrines. La suspicion de phéochromocytome a été confortée par l’analyse de la pièce opératoire qui a montré des cordons de cellules polygonales ou ovoïdes présentant des atypies cytonucléaires plus ou moins marquées. Les suites opératoires ont été satisfaisantes.<br />ABSTRACT<br />Pheochromocytoma is a tumor developed from chromaffin cells ,derived from the neural crete . 90% of these tumors  are localized in the adrenal  gland and 90% of these cases are benign. The aim of this study was to show the difficulties of histological diagnosis in a poor ressource setting. It was the case of a 49 year old woman presenting  back ache  for ten years and undergoing treatment for hypertension and diabetis type II. The result of the scanner showed a mass of the adrenal gland and the laboratory analyses showed an increased blood glucose level and urinary métanephrines .The suspicion of pheochromocytoma was confirmed by the analysis of a specimen collected during operation( Biopsy) ,which showed large trabeculaes of polygonal and oval cells presenting cytonuclear  atypies more or less marked. The follow up after operation was successful. ER -
