Le bloc axillaire est fréquemment réalisé pour la chirurgie du membre supérieur. L’utilisation de l’échographie en anesthésie locorégionale a permis une amélioration des taux de succès. L’objectif de cette étude était d’évaluer la pratique et l’efficacité du bloc axillaire échoguidée par rapport à la neurostimulation dans notre service. Patients et méthodes. Il s’est agi d’une étude transversale descriptive et prospective réalisée du 1er mai 2018 au 30 octobre 2018. Les patients ayant donné leur consentement éclairé et bénéficiant d’une chirurgie du membre supérieur sous anesthésie locorégionale par bloc axillaire ont été inclus dans l’étude. Les patients ont été randomisés en deux groupes : un groupe bénéficiant du bloc axillaire par neurostimulation (groupe NS) et l’autre groupe du bloc axillaire sous écho guidage (groupe US). Les paramètres étudiés étaient l’âge et le sexe, l’indication chirurgicale, le durée de la réalisation du bloc, le délai d’installation du bloc, le volume de l’anesthésique utilisé, les incidents et la qualité du bloc. Résultats. 80 patients ont bénéficié d’un bloc axillaire dont 40 par échoguidage et 40 par neurostimulation. L’âge moyen était de 36 ans ± 1,2. La principale indication de la chirurgie était la fracture des os de l’avant-bras. La durée moyenne de réalisation du bloc était de 5,5 min dans le groupe US et de 10,3 minutes dans le groupe NS. 95% des patients étaient satisfaits de la qualité du bloc par échoguidage contre 85% dans le groupe US. Conclusion. L’anesthésie loco-régionale sous échographie constitue aujourd’hui le gold standard en anesthésie loco-régionale sa pratique doit être vulgarisée et maitriséepar tous les médecins anesthésistes pour diminuer les incidents et complications de la neurostimulation.<br /><br />ABSTRACT<br />Introduction. Axillary block is frequently performed for upper limb surgery. The use of ultrasound in locoregional anesthesia has improved success rates. The objective of this study was to assess the practice and effectiveness of ultrasound-guided axillary block compared to neurostimulation in our department. Patients and method. This was a descriptive and prospective cross-sectional study carried out on May 1, 2018 to October 30, 2018. The patients who gave their informed consent and who benefited from upper limb surgery under locoregional anesthesia by axillary block were included in the study. The patients were randomized into two groups: one group receiving axillary block by neurostimulation (NS group) and the other group receiving axillary block with echo guidance (US group). The parameters studied were age and sex, surgical indication, duration of the block, the time to install the block, the volume of anesthetic used, incidents and the quality of the block. Results. <br />80 patients underwent axillary block, including 40 by ultrasound guidance and 40 by neurostimulation. The mean age was 36 ± 1.2 years. The main indication for surgery was fractured forearm bones. The average duration of the block was 5.5 min in the US group and 10.3 minutes in the NS group. 95% of patients were satisfied with the quality of the ultrasound-guided block versus 85% in the US group. Conclusion. Locoregional anesthesia with ultrasound is now the gold standard in locoregional anesthesia; its practice must be popularized and mastered by all anesthetists to reduce the incidents and complications of neurostimulation.</p> ER -
