De montrer l’efficacité thérapeutique de la radiothérapie métabolique à l’iode 131 au Niger, pour le traitement de la maladie de Basedow, Population et méthodes. Nous avons mené une étude prospective à visée descriptive incluant des patients en échec thérapeutique pour la maladie Basedow par antithyroïdiens de synthèse (ATS) adressés au département de médecine nucléaire de l’IRI pour Irathérapie à l’iode 131. Ils recevaient une activité d’iode 131 variant de 370 MBq à 666 MBq après une dosimétrie personnalisée préthérapeutique basée sur l’administration d’une activité traceuse d’1 MBq d’iode 131 et la mesure des taux de fixation à 2 H, 4 H et 24 H avec une sonde MINI-ASSAY type 6-20 préablement calibré à la gamma caméra Médiso Anyscan. Résultats. Notre population d’étude était constituée de 10 patients avec une maladie de Basedow, dont 9 femmes et un homme, avec un âge moyen de 50± 11. Le taux de fixation moyen de 24H était de 55 ±11% et le taux de fixation maximal extrapolé à l’aide du logiciel Kaleidagraph était de 58 ±11 %. Les activités calculées avec la fixation de 24 H étaient plus élevées que celles de la fixation maximale extrapolée. Au total, 5 patients sont devenus hypothyroïdiens, 4 euthyroïdiens et un resté hyperthyroïdien. Conclusion. La thérapie métabolique à l’iode 131 par la dosimétrie personnalisée a montré une efficacité à 90 % dans la prise en charge de la pathologie de Basedow. Elle pourrait être une alternative à la chirurgie et en cas d’échec thérapeutique par les ATS.<br />ABSTRACT<br />Objective. To demonstrate the therapeutic efficacy of metabolic radiotherapy with iodine-131 in Niger, for the treatment of Graves' disease, Population and methods. We conducted a prospective descriptive study including patients with failed treatment of Graves' disease with synthetic antithyroid drugs (ATS) referred to the nuclear medicine department of the IRI for Iodine-131 therapy. They received an iodine-131 activity ranging from 370 MBq to 666 MBq after a personalised pre-therapy dosimetry based on the administration of a 1 MBq iodine-131 tracer activity and the measurement of the fixation rates at 2 H, 4 H and 24 H with a MINI-ASSAY type 6-20 probe pre-calibrated to the Mediso Anyscan gamma camera Results. Our study population consisted of 10 patients with Graves' disease, including 9 women and one man, with a mean age of 50±11. The mean 24H fixation rate was 55±11% and the maximum fixation rate extrapolated using Kaleidagraph software was 58±11%. The activities calculated with the 24H fixation were higher than those of the extrapolated maximum fixation. In total, 5 patients became hypothyroid, 4 euthyroid and one remained hyperthyroid. Conclusion. Metabolic iodine-131 therapy using personalised dosimetry has shown 90% efficacy in the management of Graves' disease. It could be an alternative to surgery and in case of therapeutic failure with TSA.</p> ER -
