La leucémie myéloïde chronique (LMC) est une hémopathie maligne appartenant au groupe des syndromes myéloprolifératifs (selon la classification OMS de 2008). Elle est caractérisée par la présence d’une anomalie chromosomique dans les cellules hématopoïétiques, le chromosome Philadelphie. L’Imatinib a été introduit au Cameroun au début des années 2000 grâce au programme GIPAP. Il s’agissait d’analyser la réponse hématologique.<br />MÉTHODOLOGIE. 76 patients ont été inclus, 43 de sexe masculin et 33 de sexe féminin. L’âge moyen était de 41 ans (16-68 ans) et il existait une prédominance masculine avec un sex-ratio de 1,3. La tranche la plus représentée était celle de 31 à 40 ans dans 29% de cas. 52% étaient originaires de la région de l’Ouest du. Dans 94% de cas la LMC était découverte à l’ occasion de la clinique. L’intervalle de temps entre le diagnostic et l’initiation de l’Imatinib variait de 2 jours à 12 mois avec une moyenne de 1,8 mois. 88% des patients étaient en phase chronique, 9% en phase accélérée et 3% en phase blastique. <br />RÉSULTATS. La durée du traitement par Imatinib variait de 2 mois à 12 ans et 5 mois avec une moyenne de 3 ans et 5 mois. 96% avaient obtenu une réponse hématologique complète (RHC) et le temps nécessaire variait de 24 jours à 17 mois pour moyenne de 1,8 mois. La durée de la RHC était en moyenne de 2 ans et 8 mois allant de 1 mois à 11 ans et 10 mois. Parmi les 96% de patients qui avaient obtenu une RHC, 17 l’ont perdu présentant un échappement hématologique qui durait de 1 mois à 29 mois avec une moyenne de 7 mois. Malgré l’augmentation des doses, 9 patients sont restes en situation d’échappement hématologique c’est ainsi qu’on avait 84% en RHC, 1% en RHP, 12% en échappement hématologique et 3% en échec hématologique.<br />CONCLUSION. La LMC est une pathologie présente dans notre contexte. La RHC est obtenue en moyenne après 1,8 mois et dure en moyenne 7 mois.<br /> <br />ABSTRACT<br />OBJECTIVE. Chronic myeloid leukemia (CML) is a malignant hemopathy of group of myeloproliferatives syndromes. It is characterized by the presence of chromosome anomaly in hematopoietic cells, that means Philadelphia chromosome. In years 2000, we begin to make use of Imatinib in Cameroon through GIPAP programme.<br />SUBJECTS AND METHODS. 76 patients were included in our study, 43 males and 33 females for a ratio of 1.3 in men’s favour. The age of the patients ranged from 15 to 68 years, mean of age was 41 years. The most common affected group was the one between 31 and 40 years in 29% of case. 52% came from West Region of Cameroon. CML was discovered in 94% of cases on clinical grounds. The interval between diagnostic and the introduction of Imatinib varied from 2 days to 18 months with a mean of 1.8 months. 88% of the patients were in chronic phase, 9% in accelerated phase and 3% in blastic phase. <br />RESULTS. The length of treatment by Imatinib varied between 2 months and 12 years and 5 months, the mean was 3 years 5 months. 96% obtained a complete hematologic response (CHR) and the time necessary to obtain that CHR varied between 24 days and 17 months with 1.8 months as mean. The mean duration of CHR under Imatinib treatment was 2 years 8 months, extending from 1 month to 11 years 10 months. Among the patients (96%) who got CHR, 17 patients lost it, presenting a hematologic release that lengthened from 1 month to 29 months with a mean of 7 months. Despite increasing of Imatinib dose, although taking into consideration tolerance of that medicine, 9 patients remained in situation of hematologic release. So we had 84% in CHR, 1% in partial hematologic response (PHR), 12% in hematologic release and 3% in situation of hematologic failure.<br />CONCLUSION. CML is present in Cameroon. On the average, complete hematologic responses is obtained after 1.8 month and lasts 7 months.</p> ER -
