Abstract
Introduction. Au Cameroun, peu de données existent sur l’anxiété et la dépression chez les hémodialysés chroniques bien qu’elles soient les principales manifestations psychiatriques décrites dans cette population. Notre objectif était de déterminer la prévalence de l’anxiété et de la dépression, les facteurs associés et l’impact social de ces troubles chez les malades hémodialysés chroniques au Cameroun. Matériel et méthodes. Nous avons mené une étude transversale analytique dans les centres d’hémodialyse de l’Hôpital Général de Douala et du Centre Hospitalier et Universitaire de Yaoundé. Les patients ≥ 18 ans dialysant depuis > 3 mois et consentant ont été inclus. Les questionnaires d’Hamilton et de Beck ont été utilisés pour diagnostiquer l’anxiété et la dépression respectivement. L’impact social a été évalué par des questions ouvertes sur le ressenti du malade concernant sa vie sociale. Résultats. Parmi les 125 participants inclus, 64 (51%) étaient de sexe masculin et la médiane d’âge était de 45 ans [IIQ 56-32]. L’anxiété a été diagnostiquée chez 39,2% des participants et la dépression chez 42,4%. Les facteurs prédictifs de la dépression étaient le VIH (OR=3,58[1,08-11,77]; p = 0,036), les douleurs chroniques (OR= 5,52[2,32-13,1] ; p<0,001), et l’anxiété (OR= 3,4[1,5-8]; p = 0,004). Les perturbations socio-professionnelles et familiales étaient plus fréquentes chez les participants anxieux ou dépressifs. Conclusion. Les troubles anxio-dépressifs existent chez près de 2 patients hémodialysés chroniques sur 5 patients au Cameroun et impactent sur la vie sociale de ces patients. L’accompagnement psychologique des hémodialysés chroniques devrait être envisagé dès l’initiation de la dialyse.
ABSTRACT
Anxiety and depression are the main psychiatric disorders reported in maintenance haemodialysis. In Cameroon, few data are available about this problem. The objectives of our study was to determinate the prevalence, associated factors and social impact of anxiety and depression in patients on maintenance haemodialysis in Cameroon. Materials and methods. We conducted a cross sectional analytic study of 1 month in 2 haemodialysis facility (General Hospital of Douala and Teaching Hospital of Yaounde). All patients on haemodialysis for more than 3 months, and aged 18 years or more who accepted to participate in the study were included. Hamilton and Back questionnaires were used to diagnose anxiety and depression respectively. Social impact was evaluated by open questions about familial, social and professional life. Results. A total of 125 participants with 64 (51%) male and a median age of 45 [IIQ 56-32] years were included. Anxiety was found in 39.2% of participants and depression in 42.4%. HIV (OR=3,58[1,08-11,77]; p = 0,036), Chronic pain (OR= 5,52[2,32-13,1] ; p<0,001) and anxiety (OR= 3,4[1,5-8]; p = 0,004) were predictors of depression. Socio-professional and familial life perturbations were more common in patients with anxiety or depression. Conclusion. Anxiety and depression are common among patients on maintenance haemodialysis patients in our setting. Psychology evaluation and follow up should be iniatiated for them.
