Abstract

RÉSUMÉ
INTRODUCTION. Le cytomégalovirus humain peut être cause de mortalité et de morbidité chez les sujets immunodéprimés et chez les nouveau-nés. Le but de cette étude était de déterminer sa prévalence chez les donneurs de sang reçus à l’hôpital de district de Bonassama à Douala.
PATIENTS ET MÉTHODES. Il s’agit d’une étude transversale menée de février à avril 2014. Tous les sujets ont été dépistés au cytomégalovirus et ont également bénéficié des tests de VIH, hépatite B, hépatite C et Syphilis. Les anticorps anti-cytomégalovirus ont été recherchés par la méthode ELISA.
RÉSULTATS. Au total, 167 donneurs ont été inclus dans ce travail. L’âge moyen était de 31,5 ± 8,5 ans, avec un sexe ratio H/F de 5,18. On rencontrait deux types de donneurs ; les bénévoles (4,1%), et familiaux (95,8%). La prévalence du CMV enregistrée a été de 98,8%. Les donneurs du groupe O ont été les plus représentatifs avec 53,8%. Les autres prévalences ont été les suivantes : VIH 19 (11,3%) ; VHB 20 (11,9%) ; VHC 5 (2,9%) ; Syphilis 7 (4,1%). Tous les patients VIH positifs ont présenté une infection à cytomégalovirus.
CONCLUSION. La prévalence du cytomégalovirus est très élevée au sein de la population des donneurs. Ce virus ne faisant pas l’objet d’un dépistage systématique en banque de sang, des études prospectives pourront évaluer le risque lié au CMV après une transfusion sanguine surtout chez les patients immunodéprimés.

SUMMARY
INTRODUCTION. Human cytomegalovirus may be cause of mortality and morbidity in immunocompromised subjects and in newborns. The aim of this study was to determine the prevalence among blood donors received in Bonassama district hospital in Douala.
PATIENTS AND METHODS. This was a cross-sectional study conducted from February to April 2014. All subjects were screened for CMV and also received HIV testing, HBV, HCV and syphilis. Anti CMV antibodies were detected by ELISA method.
RESULTS. A total of 167 donors were included. The mean age was 31.5 ± 8.5 years, with a sex ratio M / F of 5.18. There were two types of donors: volunteers (4.1%), and family (95.8%). The overall prevalence of CMV was 98.8%. The group O donors were the most representative with 53.8%. Other prevalence’s were as follows: HIV 19 (11.3%); HBV 20 (11.9%); HCV 5 (2.9%); Syphilis 7 (4.1%). All HIV-positive patients had cytomegalovirus infection.
CONCLUSION. The prevalence of cytomegalovirus is very high in the donor population. This virus is not subject to systematic screening in blood banks, so prospective studies are needed to assess the risk associated with CMV after a blood transfusion especially in immunocompromised patients.
