Abstract

RÉSUMÉ
But . L'analyse des fibres nerveuses rétiniennes (FNR) en tomographie par cohérence optique (OCT) est de plus en plus réalisée pour poser le diagnostic de glaucome. Cette étude vise à décrire l'atteinte des FNR chez les patients traités pour glaucome à angle ouvert (GAO). Matériels et méthodes. Nous avons efectué une étude rétrospective et descriptive, dans un cabinet d’ophtalmologie à Lomé de janvier 2014 à Juin 2015. Nous avons inclus tous les patients traités pour GAO sur la base d’arguments cliniques et périmétriques chez qui l’OCT-SD de la papille avait été réalisée. Les paramètres étudiés étaient l’âge, le sexe, l’analyse des FNR à l’OCT de la papille. Etaient confirmés glaucomateux tous les patients présentant des atteintes évocatrices de glaucome à l’analyse des FNR à l’OCT. Résultats.  Parmi les 68 patients glaucomateux traités, 58 soit (85,29 %) ont présenté une atteinte des FNR à l'OCT. L’âge moyen des patients était de 53, 1± 13,5 ans [23-81 ans]. La tranche d’âge de 55 ans et plus (56,9%) était la plus représentée. Une prédominance masculine a été retrouvée avec un sex-ratio = 1,41. Parmi les 58 patients confirmés glaucomateux  à l'OCT, 56 (96,55%) avaient en commun deux critères  diagnostiques à savoir la modification de l’aspect de la papille et les déficits périmétriques évocateurs du glaucome. Conclusion. L’atteinte des FNR à l’OCT-SD a permis de confirmer le diagnostic de glaucome chez quatre patients sur cinq suivis pour glaucome dans ce cabinet. L’atteinte des FNR était corrélée aux critères diagnostiques à savoir la modification de l’aspect de la papille et la présence des déficits périmétriques évocateurs. En l’absence de l’OCT, le diagnostic de glaucome peut être orienté en présence de ces critères diagnostiques. Une autre étude décrira la corrélation entre les déficits en périmétrie bleu-jaune et l'atteinte des FNR afin de mieux explorer le reste des patients qui n’étaient pas confirmés.

ABSTRACT
Goal. The retinal nerve fiber (RNF) analysis in Optical Coherence Tomography (OCT) is increasingly carried out for glaucoma diagnosis. This study aimed at describing the RNF damage in Open Angle Glaucoma (OAG) treated patients. Materials and methods. This was a retrospective and descriptive study, conducted in an ophthalmology medical practice in Lomé from January 2014 to June 2015. Based on clinical and perimetry assessment, all patients included in this study were treated for OAG and had undergone the SD-OCT of the papilla. The parameters studied were age, gender, RNF analysis and OCT of the papilla. After the RNF analysis and the OCT. All patients with damages consistent with glaucoma were confirmed glaucomatous. Results. Out of the 68 glaucomatous patients, 58 (85.29%) had RNF damage in OCT.  The patients average age was 53. 1 ± 13.5 years [23-81 years]. The age group of 55 and above (56.9%) was the most represented. There was a male predominance (sex ratio M/F = 1.41). Among the 58 confirmed OCT glaucomatous patients, 56 (96.55%) had two diagnostic criteria in common including the change in the appearance of the papilla and the perimetry deficits consistent with glaucoma. Conclusion. The RNF damage in SD-OCT made it possible to confirm the glaucoma diagnosis in four of every five patients monitored for glaucoma in that medical practice. The RNF damage was linked to diagnostic criteria namely the change in the appearance of the papilla and the presence of suggestive perimetry deficits. In the absence of OCT, the glaucoma diagnosis can be oriented in the presence of these diagnostic criteria. Another study will focus on the correlation between blue on yellow perimetry deficits and RNF damage in order to better explore the remaining patients who were not confirmed.
